please can I have your advice on my blood test results. I take 75 mcg thyroxine for 4 days a week and 100 for 3 days. Have been on it since July 2023. I am not sure whether I am on the correct dosage as I have now started suffering from palpitations and high blood pressure.

I would be grateful for any advice. Thanks in advance.

was test early morning, ideally just before 9am, only drink water between waking and test and last dose levothyroxine 24 hours before test

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

was last dose 75mcg or 100mcg

Which brand of Levo and is it always same at each prescription

Thank you for your reply. Test was done as advised by your team.

Last dose was 100 which was too high so was prescribed 75 for 4 days and 100 for 3

FT4: 18.4 pmol/l (Range 12 - 22)

Ft4 is 64.00% through range

FT3: 3.9 pmol/l (Range 3.1 - 6.8)

Ft3 only 21.62% through range

As coeliac you are highly likely to need T3 prescribed alongside Levo

when adequately treated most people will have Ft3 at least 60-70% through range

Ideally choose an endocrinologist to see privately initially and who also does NHS consultations

Thanks. So does this mean my t3 is not converting well? Sorry not too familiar with t3.

Yes

Ideally we are looking for both Ft4 and Ft3 to be roughly 70% through range

A normal working thyroid makes 80-90% Ft4 but crucially also makes 10-20% Ft3

Very common, with other autoimmune diseases and/or after menopause for conversion of Ft4 (levothyroxine) to Ft3 (active hormone) to get worse

In the past medics got around this issue by prescribing higher doses of levothyroxine and allowing Ft4 to be over range

But it’s far better to be prescribed small doses of T3 alongside levothyroxine

This has to initially be prescribed via endocrinologist
