Before I was diagnosed as hypothyroid, not one single doctor could diagnose without a blood test. Our 'GPs in the past' diagnosed and prescribed NDTs
A number of GPs in the past could diagnose a patient due only to his/her symptoms but nowadays don't even have the tiniest bit of knowledge
I would expect some doctors to be knowledgeable but that's not the case. Even though my TSH was 100 I was told I had no problems!
Another link you might find of interest is what happens to TSH, T4 and T3 in healthy people. It doesn't stay the same 24 hours a day :
