Would this blood sheet request include ft4?

Can't tell.

FTFT will mean Thyroid Function Test. But many labs operate some reflex or cascade system - do TSH and only if that is high or low go on to do FT4. (Not necessarily the same range of values as the reference interval.)

It would be perverse to do TSH and FT3 (which is stated) but not FT4. But I wouldn't put it past any organisation to do that out of ignorance or carelessness.
