Help with lab results

Hi All, it has been a long time since I posted. I learned a couple years ago that my unwellness was not just my thyroid. I was diagnosed with SIBO and learned I had an extreme histamine intolerance and some mold in my system. We are working on all that but it is a slow process. I have been so unwell.

My functional doc has me on T3 only as I became intolerant to T4. It is not uncommon for people in my shoes become intolerant to many supplements, meds and food (one reason is they can flare histamine). Anyway, here is my thyroid situation:

At the time of my recent blood work in early November I was on 42 mcg a day of T3. My results came back:

TSH: .05 (.4 - 4.5 mlU/L)

Free T4: .1 (.8 - 1.8 ng/dL)

Free T3: 3.8 (2 - 4.4 pg/mL)

Total T3: 129 (71 - 180 mg/dL)

TPO An RDL generally runs a bit high around 12 - but we didn't test this particular time.

Doc tells me my T3 is slightly high and I should step back so I am to 37 mcg a day. I just noticed my hair has started falling out but that is the least of my problems. We will retest in another month.

Any thoughts about these lab results would be greatly appreciate - too much t3? Try to add a little T4 if I can tolerate? My Folate, Ferritin, B12, D, etc. all good in range - doc keeps a close eye on all vitamins, minerals, etc. as we try to get rid of SIBO and histamine intolerance which makes me feel so unwell. Thank you.

I don't think you need to reduce because there is space for a slight increase if anything. I expect he's going on the TSH which will always be suppressed on T3 only.

Does the T4 seem too low?? I know TSH will go real low when on just T3 but wondered if T4 is too low? Thank you so much for replying.

Yes it should be in the top quarter or third of the range (I forget which)

It has been really difficult most doctors even functional doctors Don't know about or understand how to treat sibo. I wasted three full years with a doctor who kept treating my thyroid I was so sick every day I thought I would die. Then finally another doctor did a breath test and diagnosed the SIBO and that immediately helped me understand I have an extreme histamine intolerance they go together. After that for 2 years I've worked with a functional doctor who never really did get me on a SIBO protocol. I joined the SIBO private Facebook group and That is where I learned of several doctors who have been good for other people so I'm actually going in two weeks to Cleveland clinic, a 9 hour drive! To work with a Sino doctor. I have a good understanding of what you should be taking and that includes some type of antimicrobial, a biofilm buster, a binder, and maybe probiotics. I was not willing to try this stuff on my own simply because once you start taking stuff you can launch into die off and when that happens the bacteria that is dying is letting off toxins and it can make you feel even worse! So I really need the guidance that I have not been getting!

wow, it all sounds a total nightmare. I really feel for you. just started looking into SIBO and it all looks like yet another rabbit hole.

If you get the breath test and get diagnosed with SIBO the trick is getting on a protocol and finding the right doctor. I have struggled getting the right doctor But definitely think I'm on the right track! I think there are many many people being told they have IBS, chronic fatigue syndrome, fibromyalgia any of those things that doctors tend to just label people with when they can't find out what's wrong and I think a lot of those people have sibo. I read a study that said 80% of people with IBS actually have SIBO. Crazy!
