Fissure or fistula

For the last few weeks I have been getting stabbing pains up the rear exit. Like I sat on a needle!! I used to suffer a lot with piles but not bled for about a year now. But today after large bm I bled! What concerns me is I should be having a camera look at my bladder on Saturday under local anaesthetic. This has .adequate me even more anxious., and is it a fistula or fissure or just piles playing g up. Should I tell them?

hi Edgar77, you could mention it on Saturday, but it seems like a separate issue. It seems like you should go ahead with your test on Saturday, it is understandable you are anxious about it. My friend had this and she said it was fine, easier than she thought it would be.

Sounds as if you need a different investigation for your rear end and the bleeding. Perhaps make an appointment with your GP to discuss this?

Thanks Jackie1612 This is what I was thinking, I guess I just needed a second opinion, I can't talk to my family any more, and have no friends left they just tell me to stop moaning, can't say I blame them.

Thanks Edgar77.

Please don’t blame yourself. Only a fellow sufferer will really understand.

I have found a lot of comfort in watching and listening to Tara Brach on YouTube.

Hi Edgar77, I’ve suffered with IBS and hemorrhoids for years. A year ago, I got Covid and was given Paxlovid, which made my IBS so bad for about a month. Unfortunately, I developed a fissure and now I occasionally have what you describe. The first time it happened, I told my friend (who also has severe IBS and hemorrhoids), that it felt like someone stuck a knife up my backside! She said she also gets that sometimes. Of course it’s always good to run a new symptom past your doctor, but I wanted to let you know you’re not alone in feeling this way. Hope you’re feeling better soon.

Thank you Histy. That's what I thought but it does help to know I'm not alone. I have calmed down a bit, been this morning and no blood. Just want to get tomorrow over with now. Take care.

Do consult you GP but I had fissures and opening bowels felt like passing broken glass. Meanwhile, washing with aqueous cream should help. I use this all the time now to help prevent problems

HiIf the blood was fresh I.e. bright red. The most common cause is a ruptured pile or a small vessel/tear which heals itself.

The spiking pain may be something called proctalgia fugax which often of unknown cause. A large poo may cause it though. If you're having a cystoscopy then it's worth mentioning to the consultant as it may be relevant to them, or even just to.put your mind to rest.
