Neuroendocrine Prostate cancer (NEPC) questions.
I’ve had some very concerning symptoms lately so I started looking up more about NEPC. I have some of the listed symptoms. I’ve been under enormous stress lately, caring for my mom who’s in her 90s. The stress may have cause my cancer to mutate. Last year I remember reading something on this site

Making a Case for Intermittent ADT in Metastatic Hormone Sensitive Prostate Cancer
FPC'ers, Many times, we hear people claim that you should only stop ADT if you can't tolerate it any longer, and that it will not slow castrate resistance; that it increases risk for stroke, and heart attack; that it will likely take years for your testosterone to recover; that it increases mortality

Any guidance appreciated!
Hi everyone, My Dad (77) diagnosed in January w/ stage 4 PC, mets to L5 spine. PET scan showed no other spots elsewhere. PSA was 19 at time of diagnosis, Gleason 10. BRCA negative. PSA is now at 0.2 since starting hormone therapy. Feeling good, no pain. Still works (owns his own company). Started

I am a 70 year old diagnosed in 8/21 with G9 on biopsy with initial imaging negative for metastatic disease. Underwent RP in 10/21 with seminal vesicle involvement. A few sampled nodes were negative. 5 months after surgery PSA climbed to 0.39 from 0.016 in first post op test. PSMA PET revealed a solitary

How long does SBRT to two lymph nodes continue to lower PSA?
Just wondering…does SBRT continue to drop PSA for any length of time? Or is it five sessions and it is what it is then? Curious if I can see further PSA reduction after 5 sessions to one node, then 5 to a second node. No prostate. See the treating RO in a couple of days, but thought I would ask

Trying to make sense of things
Can anyone explain why some people have Gleason 8 PC, high PSA and have metastatic disease, yet some with Gleason 9 and 10 have low-ish PSA (as far as PC goes) and no obvious metastatic disease (confirmed by PSMA)? I am trying to understand this damn disease and sometimes there doesn’t seem to be any

Ct scan of abdomen and pelvis and full body bone scan?
Hello, I have been advised to get a ct scan of abdomen and pelvis with and without contrast, along with a full body bone scan prior to my next appointment with my urologist. PSA has been undetectable since completion 9/9/20 of image guided radiation to the prostate bed and lymph nodes with a 6 month

Not great news for hubby today. What is next?
Just updated my husband's profile so you can see his journey from 2016. Oncologist today said to continue the Abiraterone for another month and see where we are at with PSA results. Was up again today. He did have 6 rounds of targeted radiation last month, so would that cause PSA to go up. We were hoping

Rising PSA
What are the most Accurate scans for detecting hot spots of cancer? Husband has had several PSMA scans and CT with contrast yet nothing shows up. He had one lymph show up in 2017 on an Auxium scan and it was radiated. 7 years later PSA rising… up to 14. Yet scans show nothing definitive. CT with

My experience with Docetaxel
I know everyone doesn’t respond the same but here is my experience and what I went through with 10 rounds of chemo. Before I started I had already been on Lupron for five years, I had radiation to multiple bone Mets, took zytiga/xtandi, had Provenge and did a clinical trial. October 26 2023 I started

What comes after, what comes next?
I have posted a few times about my dad. Stage 4, mets to 8+ bones at time of diagnosis, PSA at 137 (10/22.) Lupron twice a year, Bicalutamide, failed at 7 months, Xtandi failed at 4 months, currently on Zytiga. We met with my dad’s oncologist on 4/24. My Dad’s PSA dropped from 2.47-1.46 and pain has

PSA uptick
Quick background, my dad was diagnosed with metastatic PCa in 2018. Initially treated with docetaxel + ADT. His PSA has been at/near undetected (<0.01-0.02) for several years, but recently there has been an uptick to 0.08 (similar on a recheck, so not lab error). Obviously still low-level, but concerning

Phase 1 trial shows promising results for fractionated dose of 225Ac-J591 in PCa treatment
The fractionated dosing achieved promising results with 95% of patients experiencing any PSA decline and 71% achieving a PSA50 response. In contrast, the multiple cycles cohort saw 72% of patients with any PSA decline and 28% achieving a PSA50 response...fractionate good! https://www.urotoday.com/conference-highlights

177Lu-PSMA-617: Trial shows it's more effective than changing ARPIs
"Patients who enrolled in the trial were assigned to either a change in ARPI, for example from abiraterone to enzalutamide or vice versa, or 177Lu-PSMA-617, a prostate-specific antigen (PSMA)-targeted radioligand therapy. This analysis found that treatment with 177Lu-PSMA-617 prolonged radiographic progression-free

radiation treatment delayed
2 years since prostatectomy. Psa still non detectable. Will get it checked again in 6 months or 1 year depending on urologist suggestion. I feel blessed since standard protocol was radiation to the pelvic area had 6 Month Lupron shot since they found positive pc in limp nodes and tissue extracted.
