Any treatments left?
He has been on
abiraterone
, Xtandi twice, zytiga, Docetaxel, radium-223, BAT and now he is about to get his 8th treatment of Jevtana. He is 77 years old and is tolerating the Jevtana pretty well. His recent bloodwork shows his PSA is a little over 1000 and his phosphates are just over 300.

My husband's experience with Xtandi
In April he was put on
Abiraterone
+ Prednisone, and after three months of continued rising PSA the doc switched him to Xtandi + Prednisone. The
abiraterone
was not so good--lots of fatigue--but the Xtandi was TERRIBLE. Husband had zero energy: literally taking a nap would exhaust him.

A high Neutrophil-to-Lymphocyte Ratio [NLR] is a poor prognostic factor for CRPC patients on Abiraterone [Abi] or Enzalutamide [Enz].
New study below [1]. I posted a review of the NLR-PCa literature 4 years ago: https://healthunlocked.com/advanced-prostate-cancer/posts/134756331/inflammation.-1-neutrophil-to-lymphocyte-ratio-nlr There have been similar studies since then. Yesterday's 2 month PSA reads vs Sept 18th. A 0.051ng/mL Delta increase. Should I be concerned?? Seee my newest Med/Onc this Friday.
I sent my newest Med/Onc a msg with some of my ADT (
Abiraterone
250mg, Lupron(Eligard), prednisone 5mg 2X/day) He evidently did not see them as positive and ordered me; 1) CT scan abs/pelvis/chest 2) NM technicium bone scan and new set of labs, including TSH(thryoid).

Having SABR radiotherapy to 61mm met on my hip
The issue is that it hasn't shrunk so that is why they will do the SABR treatment, ie the
Abiraterone
is not working on the bone met. Is it possible to remove that size of tumour with SABR?

Abiraterone vs Enzalutamide + Radiation
One of my oncologists suggested that either
abiraterone
or enzalutamide could be used but noted that prednisone must be taken with
abiraterone
, and it is well known that prednisone is immunosppressive.

Our Saga so far...and an observation about quality of care
Five months of
Abiraterone
and Xtandi have left him almost completely drained of energy. On the old ADT he kept very active, working outside every day, hiking, etc. but now he can't do much of anything.

Zoladex and Enzalutamide?
We took a long shot and asked if he could be placed on
abiraterone
along with this as the NHS are currently offering this in place of chemo but they refused but have offered him enzalutamide as an alternative. He also asked if he could be placed on zometa but she refused to put him on this.
