If your PSA has been undetectable for 5 years, I would have a PSA done every 3 months.

Just annually. That's what I did after my PSA hit nadir after RT.

6 years clean on current 6 monthly testing. All less than 0.1 until last one which must be from a changed test. It was less than 0.025. Could probably go annual anytime.
