I'm 6 months post sct but my platelets are taking a long time to recover( only c40) so I'm not allowed ibuprofen or aspirin. I use paracetamol for most pain however I sometimes get muscle spasms in my neck/shoulder which I found ibuprofen treated. Does anyone know of an alternative painkiller that may help?

I would wonder about using a different type of medication to deal with muscle spasms. Perhaps one of the muscle relaxers used to treat muscle spasms would help (e.g., cyclobenzaprine). I do not know whether these meds are contraindicated post-SCT. That is a question for your MPN care team to address.

I imagine that your team at Bristol would be able to help, either with a different painkiller or, as Hunter suggests, a muscle relaxant. In your situation, I would stick only to advice from the transplant team.
I get short bouts of muscle spasms in my back which I shift with tiny doses of diazepam.
Also I go for deep oscillation therapy when the spasms are bad.
It is a kind of very gentle deep massage.
Post SCT It took years for my Platelet count to creep up to around the hundred mark where they remain. I don't know the reason why they aren't anywhere near in range and never bothered to ask. But if I remember I will at my next review in September.
It's strange because my Sister as my donor regularly has a healthy Platelet count slightly outside upper range but not MPN related.
Regarding alternatives to Ibuprofen maybe your chemist might help but I always seek an expert opinion on these type of questions from my Transplant consultant or nurse specialist.
