Hi Lainey1959 - perhaps members in the forum may be able to help if you say in what way you're hoping to support your carer. Is it in regards to any benefits or employment for example?

I don't believe there is such a template for your carer as regards PIP, but thank you for editing your post to make your query clearer.

As everyone (even with the same disorder) will experience different difficulties then there is no such set template.

PIP is not about any diagnosis, but rather how your disability affects certain activities of daily living &/mobility. Please see the activities/descriptors that are looked at with PIP:

cambridgeshire.gov.uk/asset...

Give a couple of recent, detailed examples as to the difficulty you face for each applicable descriptor, i.e. when did it happen, where, what happened, did anyone see this, & were there any consequences to attempting/doing an activity?

Say if you can't do an activity 'reliably,' i.e. safely, to an acceptable standard, repeat as often as one would reasonably expect, or if it takes you much longer than someone without a disability. See this mentioned at the end of the link above.

Try keeping a diary for a week or so either for reference, or you can send it with your claim form. Just complete a few questions at a time, & you can always add extra pages at the end. Just ensure you put your name & National Insurance number on each page. You can also ring the PIP enquiry line on 0800 121 4433 to ask for extra time to complete the form, & you will be given another 2 weeks without question. Keep a copy of everything, & get a free Certificate of Posting from your Post Office when sending the form off.

Your carer could send a supportive letter describing the difficulties they've seen you face so long as they relate to the above descriptors, or say which activities they help you with, & why.

Remember it's about the help you need even if you don't always get that help. PIP is also about how you are the majority of the time, i.e. over 50% of the time.

Hi there, i'm afraid only your carer will know what you are dealing with on a day to day basis. It would be unethical I would say for anyone to write out such a template not been your personal carer I'm afraid.

All your carer needs to do is explain what they themselves have to help you with on a day to day basis and how they have personaly seen how your illness effects you and your needs xx
