I hope this finds everyone well. In 2019 an ultrasound showed fatty infiltration. I went on a strict diet, exercise program (i was already in good shape). In 2020 i had a Fibroscan which showed normal kpi of 4.7 (F0). In Nov 2021 i had an MRI of the liver which showed everything normal (not even hepatic steatosis). I just went for my annual checkup this week, got full bloods done and my AST was 18, ALT 13. Ratio is over 1.38. Rest of the bloodwork is great - bilirubin 0.6, albumin 4.7 Platelets are 220. Is there any reason for concern here given the ratio? I keep reading conflicting opinions on what exactly that means. Given my recent Fibroscan, MRI, etc am i overreacting here?

When the numbers are that low (and all the other numbers you mention are good) there is nothing to worry about. The ratio is not calculated when the levels are that low because they are in healthy range. Doctors only get concerned when the ALT and AST get over several times the "upper normal" range. Your numbers are six times below any worry level.

Of course you should listen to doctors not internet message board posters, but I am 99 percent sure they will say exactly what I just said. Good luck! You should be fine!

Thanks so much for the response Ken. That's what I'm thinking as well - waiting for a call back from my doc in the meantime. The ratio conundrum is a bit unclear from what I've read. Seems that most agree with your sentiments but others are a bit of a gray area. Thanks again for responding. It's helpful to get another viewpoint.

There is no grey area when the numbers are that low (in the teens). I don't mean to come across as rude, I am just telling you - as a non-doc - your doctor will tell you the numbers are terrific. Good luck.
