It is my opinion, since I do not have any supporting data, that following could be reasons for higher prevalence of diabetes in urban areas---

-Rural economy is largely agro based in India.Physical work is still common and obesity is much less in rural parts.

-They consume relatively less processed foods. They definitely eat healthier food than urban folks.

-Infrastructure being poor and vehicle density being very low, walking long distances is quite common in their daily routine. No Uber or Ola there.

-Pollution of all kinds is much less and life goes on in a very routine fashion leading to lower stress levels as compared to urban folks.

-Literacy , awareness and access to medical care are relatively less in rural parts.I suspect many cases go unreported.

That saiad, sad part is,other health problems are rampant mostly due to ignorance, lack of access to medical facilities, unaffordability etc.I mean, diabetes may not thrive in rural parts, but general medical conditions are not good in those areas.
