I’m confused about B6.

is the advice with Co-Careldopa not to take, at all.

Or to take after Co-Careldopa has been absorbed.

my recent blood tests showed me deficient in B6.

Because a deficiency is found, I would suggest starting B6 (P5P) supplementation (max 3 mg) eventually through B-complex or multivitamin. After 3 months of B6 suppletion you have to do a new test. Always wait about 3 months for a meaningful check of your B6 level related to the extremely long half-life of a month of B6. The impact of Carbidopa on your B6 levels is likely much less than previously thought. Through HU, I have gathered a decent database of B6 test results from PD patients. Some from PWP without supplementation, and some with.

Preliminary results show that measurements without B6 supplementation usually indicate a (often severe) vitamin deficiency, so you're not the only one... Strangely, one test result showed an overdose. However, ALL measurements with B6 supplementation, even at very small amounts of >4mg, indicate an (usually severe) overdose, regardless of whether it's P5P or pyridoxine intake and even the amount of Carbidopa intake. Shockingly, out of the 20 test results, none fell within the desired minimum and maximum reference range. This means that NOBODY had their B6 balance in order. Even after four tests, I also still haven't achieved a balanced B6 level. I hope to achieve this by the next one early January.

If you or others on HU have B6 test results, please share them with me, either publicly or through chat. Please include the following information:

- Test result with stated minimum and maximum reference range

- Whether or not B6 supplementation was taken

- Amount of B6 supplementation in mg (including through B-complex and multivitamin)

- Form of B6 (pyridoxine or P5P)

- PD patient or not

- Daily dosage of C/L in mg or separate Carbidopa dosage, if applicable

- Date of the test result

- If possible, also indicate if there is peripheral neuropathy or symptoms resembling it.

The more data we have, also if the values are normal, the better we can identify correlations and potentially provide clear advice on B6 dosage. Thank you all in advance! 🍀

I had 108 ng/ml in three months ago. After three months of b6 stoppage its 33 ng/ml ( I think still on higher side) . But the possible toxicity has aggravated my symptoms a lot

Thank you! I already had your first result in my file. There is still missing the information about the possible amount of carbidopa / levodopa that you take daily.

I take 100/25 ER , 3 or 4 times daily.

Your last result looks fine. You have the only result that falls within the reference range! Although you probably haven't had B6 supplementation for 3 months, I estimate that due to the long half-life mentioned, you still had about the availability between 5 and 15 mg B6 per day in the blood sample used . Only in a few months will your initial high dose of 100 mg be virtually worked out. You will probably need B6 supplementation again in the long run, you could start with that soon, but I do not recommend more than 1 to 2 mg at this stage. After 3 months, check again what this does to your level. Good luck.

I had a B6 test done on August 3rd of this year. It came back normal (13 ug/L, ref. range 3.4-65.2). I was taking 25 mg of P-5-P daily, up until about 2 weeks before the test, then I stopped. Now I'm taking a B-complex with 10 mg of P-5-P once or twice a week. I still have lots of peripheral neuropathy, which has been a symptom since the beginning of my PD diagnosis. I currently take 25/100 mg of carbidopa/levodopa ER, 4x daily.

Hello Doohat , what are the symptoms of your peripheral neuropathy?

It's mostly numbness in my feet and calves. Also my right arm is affected. I get carpel tunnel syndrome whenever I use anything that vibrates like a weed whacker. Seems like I have reduced feeling through out my whole body.

3 mg of B6 is woefully inadequate. Do not be swayed by the words of an individual who had a very unusual reaction.
