was just wondering why OandV go together is it possible to just do Obintuzumab infusions to get good results? Thanks. Everyone is so encouraging I appreciate it

Combining two (or more) drugs gets you to undetectable Measurable Residual Disease (uMRD) with higher efficiency, faster, plus reduces the risk of resistance developing. This means that you are likely to have a longer remission, which means you should be able to repeat the same treatment should your remission end.

If you observe independent data from studies with Obinutuzumab and venetoclax, two measures of importance stand out with each.

With obinutuzumab there is a tendency for immediate efficacy with a manageable safety profile. however, as a mono therapy the durability is approximately two years before relapse (short durability).

With venetaclax there is also immediate efficacy even more so than obinutuzumab in such a way that tumor lysis is difficult to manage. Data does indicate however, that durable remissions (4-5 years) with venetaclax can be seen much more than with obinutuzumab.

In the medical community discussion, it is said that the obinutuzumab is very efficacious in the peripheral blood whereas venetaclax goes even deeper in the bone marrow giving reason to the durability aspect.

One reason for combining the two drugs as purposed in the trial that I participated in makes sense, in that the obinutuzumab clears the peripheral blood effectively and safely so that the venetaclax could clear the bone marrow with lower TLS risk achieving Umrd and durability.
