If your over 50 and need a stem cell transplant it most likely will be low intensity. It also goes by mini-transplant or RIC transplant.

It is NOT a standard stem cell transplant. all your cells are not replaced. you keep most of your own blood cells and have donor cells added to fight your cancer cells.

the reason i bring this up is because transplant statistics in general are old and don't differentiate between types of transplants and most of these statistics are from prior to targeted therapy.

a reason to at least consult about a reduced intensity transplant is because the search for a 10/10 donor takes time..

that is the most important thing. only 30 percent of relatives are able to be a match.

the hospital gives your parameters to a search provider and the search goes worldwide. Only 60 percent of caucasians can find a 10/10 match. the longer your in the system for this the more likely they will find some in case you need the

Seems to be movement away from HSCT transplants in CLL, over the past few years as candidates with high risk disease have better outcomes often on a series of small molecule inhibitors.

Also CAR T cell, and CAR NK, while still very experimental, is improving in safety and efficacy...

GVHD has been a lingering issue in transplants in CLL, but now that Imbruvica (ibrutinib) has been approved to treat it, perhaps we will see RICs return... but still there is an age and comorbidity issue.
