Ibrutinib and Cll

I’m concerned that my husband who’s on ibrutinib for Cll has a raised liver count ,

This is the 2nd time he was on three tablets a day which was reduced down to one after he’s liver went back to normal he’s now back on two tablets and he’s looking ver coynt has increased again ,Will he still be able to take this life saving drug

I just read this report with my Espresso.. it isn't a good example. This patient had Richter's transformation and RCHOP, BEAM, AND RICE... and a transplant.

I'm franklly surprised he had any liver function left after those...,

Imbruvica (ibrutinib) simply pushed him over the top to liver failure, I don't think it can be painted as a problem on a single case...

Worth mentioning to the doctor certainly...

Zydelig (idelalisib) the other current BCR inhibitor is black boxed for hepatotoxiciy, and when on it my liver counts were all over the map...

As Chris has said above, he doesn't think the liver failure in the case presented here was Ibrutinib induced.

Here is the statement within the conclusion of the presentation:

Our patient had normal LFTs before initiation of ibrutinib, negative workup for infectious and autoimmune etiologies of liver injury, and two liver biopsies consistent with drug‐induced liver injury, leading to the conclusion that ibrutinib was the cause of his liver injury. To our knowledge, this case is the first reported case of ibrutinib‐induced severe liver injury. The exact pathophysiology of this liver toxicity is not yet known and requires closer monitoring and investigation in both the phase 4 setting and current clinical trials with ibrutinib.

You will note that the patient had a lot of treatments before he had Ibrutinib, I do not know that your husband's situation is--if he has had treatment before having Ibrutinib and so exposed to chemotherapies.

We can't know if Chris' surmise is correct, or if the investigation as it goes further will prove that Ibrutinib can be the culprit.

I just hope you will talk with your CLL specialist (if you have one) or will get a second opinion from a Specialist if you do not. Also, of course let your Dr. know about this one case that has not been proven, but has been published.

Please let us know what your medical team is saying about possibilities of getting back on Ibr. or another treatment that is possible for your husband.
