This happens to me occasionally, at first I was worried that I wasn't getting the medication as normal and, like you, find that the capsule is empty. What I have noticed on inspecting the capsule is that the holes that have been pierced are bigger than normal, when it's a smaller hole it vibrates well, when it's a bigger hole it doesn't.

I don't notice any difference in my breathing so I believe I am getting the full effect of the Spiriva even when it doesn't vibrate normally.

I try not to be too heavy handed when piercing the capsule, I think if I press the button hard that's when the hole is bigger.

This is my experience, I'm interested to see if others have this problem.

I think that has a propellant doesn't it, I can't use inhalers with propellants but I manage well enough with the breath activated ones.

I’ve. Been on spiriva respimat for years.

It’s a spray,takes a bit of practice,but no powder residue.thank goodness

I had this problem. I also had the feeling that sometimes, especially if I was unwell (when I needed it most) it sat at the back of my mouth on my tongue. The nurse changed me to Spivira Respimat aerosol, suits me much better. So much easier to get a good inhalation.

Spoke to my pharmacy this morning she suggested I ask gp to change to spiriva respimat

I use the respimat and find it very strange. I keep it in my mouth the whole time when turning and clicking as the dose escapes whenever if likes.

Turn and click with the lid closed. Then open lid, put in mouth and breathe in as you click the button. That way nothing escapes

when I open the lid my heavy handed ness presses the release button and it all flies away
