It’s been 12 days since my first Pluvicto treatment and I’m wondering if it might have or still be seriously challenging my gut bacteria. Constant lower GI discomfort/pain with much more gas than usual and exceptionally rank odor for flatulence and feces(sorry if that’s TMI).
Don’t recall getting any information regarding the possibility of this sort of situation. Kinda reminds me of in the past when I’d get a rough course of antibiotic therapy.
I took 4mg zofran and it does work. How’s your fatigues?

Hard to know how much of my current fatigue is a side-effect of the Pluvicto or how much of it is exacerbated by what I had going on just prior to and after the treatment. I’m thinking my 2nd. round should be less encumbered with other stressors; but for now it’s not much worse than while I was taking Zytiga.

Green tea has been my morning drink since just after diagnosis. But I did get briefly away from it during most of the 1st 10 days following treatment.
4 mg zofran helped me through it after my PCP decided to give it to me (MO and RO staffs not concerned with "minor" SE)! Also, if the first week is bad for pain: an MO at MSK recommended to me a low dose of a steroid, dexamethasone, starting morning of injection and diminishing to nothing over 7 days. I had no SEs (well after my Primary Care got me zofran), although I did have to lower the steroid dosage as I got better to keep from over doing it because I felt too good! LOL
“felt too good”…I really love that! That’s something you don’t hear much from those of us who’ve qualified for the Pluvicto club! Thanks for sharing your insights regarding the side effects. I didn’t experience much pain immediately after my first round, beyond the lagging lower GI distress. But I’ll keep the steroids prophylaxis in mind in case things change as I continue on.
Your duodenum has tissue that expresses PSMA, so it attracts the radioligand and gets messed up for a bit. This could explain your symptoms.

Five days ago, I had my second Pluvicto. The first treatment was rough with constant nausea, vomiting, and diarrhea for two weeks or so. By contrast, this second one has been a "breeze." Only one bout of naseau and vomiting and two days of diarrhea.What I learned, for me, everyone is different, is I started taking a probiotic for stomach and colon twice a day, am/pm. Also, probiotic yogurt or similar. I continue doing both.
The Dr. had prescribed Prochlorperazine 10mg for naseau and vomiting, which I took intermittently during my first Pluvicto dose. This time, I have it set on my clock and take it three times a day without fail.
I also take 5mg of Prednisone, which helps my joint pain after last years nine bouts of chemo kicked me Hard. Hope this helps.
