Prostate Cancer Ablation and how does it work.

Prostate Cancer Ablation? I read about it on the internet, has anybody here have had this done. I read some great news about it, but not much is talked about it here. I really don't know much about it but it sounds interesting.

The reason it isn't discussed in this forum is that thermal ablation of the prostate is inappropriate for men with advanced prostate cancer. It has been used to ablate metastases in places where radiation may be problematic. There are clinical trials utilizing cryo, HIFU, or PDT of bone metastases.

HIFU was used in a study just before 2010(?), in California, overseen by the FDA for men that had elected to have radiation of prostate cancers and had biochemical recurrence.

After radiation surgery isn't viable. HIFU worked!

Thanks. The advances made since 2017 when this was compiled and since the link I supplied --not only HIFU but also FLA --the advances made in four years present time would take for decades if not longer in our grandparents time. My grandparents were born before the automobile. Three saw man walk on the moon.

Avoiding radiation is smart as ionizing radiation is a known carcinogen.

You can imagine whatever "advances" you like, but the data show there are better ways to conduct salvage after radiation failure.

Ionizing radiation is a known therapy - avoiding it is foolish.

Hey T_A!

I do not imagine things. I have a creative mind that deals in facts, factors and compilation of data.

For instance--why on earth was I advised by a couple uros early on to avoid radiation treatments because of my age and excellent health as it can lead to future cancers and even fecal incontinence? That wouldn't be fun. I thought it quite prudent to follow their advice as in addition to the data they had seen this happen to patients. I'll let someone else spin that roulette wheel--unless it is the last option I wish to exercise.

I do appreciate much of the information and studies you share. I tend to stay away from four year old things about treatments if possible. If President Jimmy Carter had had his cancer metastasize in 2011 rather than 2015 would he be alive today? Four years does make a difference. Keytruda was first approved by the FDA in September 2014 for melanoma. Since then it has received approval to be used as a treatment for more cancers.

Unfortunately, it takes several years to prove a therapy works, especially for prostate cancer. Keytruda was proven to work for melanoma in a trial that began in 2015 and ran for 3 years. Melanoma has a much shorter natural history than prostate cancer.

That is sad that you " tend to stay away from four year old things about treatments if possible." Docetaxel, cabazitaxel, Xofigo, Zytiga, Xtandi, and Provenge were all approved for advanced prostate cancer before 2017. The curative therapies for localized prostate cancer have been around since the 20th century.

Once a therapy is proven by a clinical trial, it stays proven. Do you imagine that it becomes less proven over time?

Just as there are advances in automobiles and planes, there are advances in treatments and drugs. I want the very best if needed, not the 20th century if in 2021 something new is here. Remember when a car was started manually by a crank? Why would I want that technology when I can drive a Lexus or Tesla?

When I was a young man I heard a man say that there was no need for a toilet in his family homestead, a large farm passed down through the generations. If the out house had been good enough for his father and his father's father, it would certainly be good enough for him and his family.

A dated treatment or drug will still work. It was the best of it's time. One can still shit in the outhouse. Not much fun in January! But the the best place to shit in it's time! Beats trying to dig a hole in frozen earth to bury "it"!

I had focal treatment by HIFU 2 n arf years ago against the recommendation of one highly regarded centre due to tumor size and possible spread. I rolled the dice and seem to have come good. 2 biopsies later no sign of the beast. No ADT, just a day as as outpatient, and after a long recuperation I can say that I'm firing on all cylinders. But I will keep looking for any recurrence ...

I had laser ablation of about 25% of my prostate. Some urinary urgency and ED for about a year, but now 4 years later everything is back to normal and PSA levels are good.
