The scans were good in spite of a slight rise in PSA pelvis/amdomen showed no progression and bone scan showed no new mets and no progression. He agreed to start casodex and feels I should get a long run with it along with lupron 3 month. The bladder issue is an overactive bladder and taking meds for that but side effect is diarrhea, a real pain in the ass. Now I can join the women in wearing pads since frequent trips to the bathroom is not always possible. Do most of you take the casodex, do not know what strength it us yet as I have not received from mail order, with your breakfast and is it more effective with a higher fat meal ? It says take it a half hour after you have voided but why ? Thanks for the information and hope all stay safe.

If you have been taking Lupron, there is no reason to take Casodex. That is very old school. You should be combining the Lupron with Zytiga instead. The combination increases survival and slows the time to castration resistance.
It is Zytiga, not Casodex, that is absorbed better with fat. But it's better to take the full 1000 mg Zytiga dose with no food.
Thanks but cannot afford the new expensive stuff and zytiga and research does prove that taking less with fat gives you a boost. The dr wants to try this first then go to zytiga. Others have used this combination for a number of years before moving to the next drug and peoples reaction differ. He feels since I have gotten almost 2 and a half years so he feels my responce would be good. I can use the other in the future, I think magnussen has been on it for a few years. Right now dealing with a overactive bladder meds that cause some unpleasant side effects, buying stock in depends.
If you cannot afford the full dose Zytiga, maybe you can get by with 250 mg abiraterone (generic) with a high fat breakfast. Casodex absorption is not affected by meals.
One can find an anecdote to support any theory. That's why doctors rely on research across many patients rather than trusting in what some random person on a patient forum said worked for him.
Using abiraterome earlier has a much greater effect than waiting. You are making a mistake in judging the effectiveness of a drug by your PSA. Men who use abiraterone live longer and avoid castration resistance longer, even though PSA may be undetectable with ADT+Casodex. PSA is not prostate cancer - you want to treat your cancer, not your PSA.
Thanks for this clear and sound advice you offer T_A. Add on question: does this mean the evidence supports that ADT + Zytiga is superior first line to ADT + Xytiga or similar AAR drug?
I have been working with the doc on second line and he agreed on that path due to good recent scans. If it fails then move to next stuff which would be zigtia.
I'm sorry that he is ignoring the practice-changing findings of two major clinical trials and the NCCN recommendations. Perhaps you may want to get a second opinion on this.
Appears to be a typo. Are you asking whether ADT+Zytiga is superior to ADT+Xtandi as a first-line therapy? If that's what you meant, a recent study found that the total time during which they work is longer for Zytiga -> Xtandi than for Xtandi -> Zytiga.

Congrats on no new Metz or progression . Good luck on casodex and Lupron ..T_ A says Zytiga and he does know what he speaks of . Maybe there is a way to get it payed for? I sure would hate for $ to halt you from what’s best ? I Did a similar combo with Lupron and Tak-700 . although it failed for many, or many couldn’t take the extreme side effects ,it has miraculously continued to work for me over five years now . I had major problems with diarrhea, and was loosing weight.. Te best treatment was cholestyamine, one packet twice daily. As to the etiology, the first item that made a major difference was stopping the metformin. The second item was the mg, that was in the calcium that I was taking, 1200 mg/day. Stopped it and the diarrhea is now gone. Now, I will reintroduce the items slowly, paying attention to dose and BM. Yes, the weight is coming back.
Good you tound the problem, I assume you got the weight back. My bladder problem got critical last night with a call to 911, I was in so much pain could was hardly stand up straight, they had to put a catheter in and drained 2 liters of pee. Have to see my GP tomorrow and a urologist this week, keeping the tube in for a few days until I see him. Now is my chance to really wash out the kidneys since my creatine level was above normal. First time I have ever called 911 but the pain level was near 10. Could not drive in that condition so my friend who shares my house and life had to call uber to get me. Luckily I had some anti nausea pills left from chemo 2 years ago that took care of the nausea so I did not barf. This morning is better the pain is gone and just have to drink and rest up, the 5 cats will keep me company.
I was on casodex for 5 years before it started to fail. I did not have any side effects. It was a long time ago but if memory serves, Casodex comes in 50mg tabs. Some how I think I took 150mg daily. It is an older "-lutimide" drug but I believe it is a good starting place.
I agree with TA about the effectiveness of Zytiga versus bicalutamide, however, I have been on the added bicalutamide 50 mg for 3 1/2 years and counting after Lupron alone was effective for 4 years. Scans at MSK remain clear( Gleason 9- numerous Mets) Not recommending- just reporting my individual experience.
That was my way of thinking, if it fails then move to zygtiga. He feels I have done well with lupron so should do good with casodex , been on lupron for 2 and half years and has worked. The side effects have been nothing except the hot flashes so will take both, the generic casodex has little side effects from what I read, when do you take the meds ?

I've had multiple stents (in and out every 3 months) from my kidney because of a stricture in my urinary tract..... It was really a nothing sandwich... for several years.... No longer have a stent.....
BTW if you're going to use Diapends.... buy them by the case from Costco (best price)....
