Question regarding PSA

My previous PSA have been 5.6, 5.8, 5.4 and now 6.0 they have been attended every 6 months over the last 18 months. Urologists says check again in three months time with another mpMRI. Last mpMRI showed PIRADS 4 and a growth in tumour of only 0.3 mm.

On diagnosis Gleason 6 with 80% volume in two cores 5% in one other. Does this sound like a good plan for Active Surveillance, checking again in three months time, rather then 6. I’m 61 years old

Hi Ian Everything you wrote "sounds" OK, except for Pirads 4. Ask for a second opinion by a different doctor on your mpMIR to confirm and then look to see what your next mpMRI shows. Meanwhile, keep enjoying life.

Also thanks for this invaluable site. I appreciate you taking the time to respond. I have sought a second opinion from Professor Stricker in NSW. He took a look at my results, sent my histology off to another lab as well. He thinks Active Surveillance, if actually performed actively, to keep a close eye out for any changes will be fine. We spoke also about Brachytherapy, seed implants. But my urologist here in Tasmania doesn’t feel this is necessary and I’m happy with that.

There doesn’t seem to be any strict protocols for Active Surveillance, what I was getting at is should I have my PSA monitored every 3 months on a regular basis or is 6 monthly fine, only check sooner if it has a significant change. So many things can effect the range.
