Thanks Paul - interesting to see how others record - Have tried a number of different table layouts - agree concise is helpful.

In the past I have also tried rating symptoms out of 10 to get an overall "wellness" record. Found taking 5 symptoms the easiest format. e.g. energy / sleep / headaches / emotion / processing.Then each effectively worth 2 /10 of day score

Symptoms are also important but the signs are essential as they are not subjective.
