Help with result please - endo treating by TSH only!

Hi there. Please can someone help with these results. My endo keeps dropping my T3 and wanting me to add more T4 via NDT. He originally suggested I add in 50mcg levo even though he said in his letter that I had previously not tolerated it, hence the NDT. I note from my medical records that I was diagnosed with Dysphagia goutre back in 2001 and in 2010 a goitre, swoloen neck and small thyroid was mentioned with question mark next to it. I was never told of these diagnoses including impaired glucose tolerance, moderately enlarged heart or the fact I had Hashimoto's. The only result I can find was 29 which was just within the range. I did have positive TPO at one point but in 2017 it was recorded as negative.

I also have benign essential tremor and OSA in neck.

My Vitamin D was recorded as 130 in 2019, Ive not had ferritin or folate tested recently so intend to get doc to do these. Any advice is welcome. My endo is unfortunatey TSH obsessed and not concerned with T3 result 3ven though I have to go to alot of trouble to get this via the surgery.

Also have dreadful tinnitus which I am aware can be helped when thyroid results are optimal! I also found this link on this site and will be dorwarding to GP and endo. My GP has refused T3 twice and never communicates with endo, just leaves me to it!

FT3 6.3 (3.1 - 6.8)

FT4 0.05 (12 - 22)

September 2020

TSH 0.04 (0.27 - 4.2) - 40mcg T3 - 1/2 grain NDT, moved to 1 grain of NDT as still had symptoms

FT3 3.6 (3.1 - 6.8)

FT4 0.5 (12 - 22)

December 2020

TSH 0.03 (0-27 - 4.2) - 40mcg T3 +1 grain NDT

FT3 3.9 (3.1 - 6.8)

FT4 0.5 (12 - 22)

Now on 30mcg T3 and 1.5 NDT.

Having blood test on 20th and chat with endo on 26th.

Are thyroid tests all done as early as possible in morning before eating or drinking anything other than water and last 1/3rd of daily T3 and daily NDT approx 8-12 hours before test

As you have Hashimoto’s are you on strictly gluten free diet

If not get coeliac blood test done BEFORE considering trial on strictly gluten free diet minimum 3-6 ,months

What vitamin supplements are you currently taking

These tests were taken 9 - 9 30 without any food and at least 12 hrs from dose of both NDT and T3. I am not strictly gluten free but am about to try as hubby is also doing same.

I am on Thiamine, B1 spray, Magnesium choride liquid, niacin, pantothenic acid. I do not think the thiamine is making much difference even at 350mg (5'greens brand). In fact Im not sure any of the supplements are, its really difficult to know and sifficult to work out which co-mordidities are thyroid related or not. I have OSA of neck which might or might not be cauding my tremor. I have had Datscan/MRI done which thankfully came back negative for Parkinsons. Tremor has been ongoing for at least 15 years evwn whilst on levo. Was left under medicated on levo for probably 30 yrs between 50-75mcg

I was on NDT only for a while but could never get to the 'sweet spot'. Just wondering why my FT4 kedps dropping, but also think goitre could be an issue now as getring hoarse voice and SPO2 drops to 76% at night (according to gadget I use). They say I have mild sleep apnoea so have been turned down for CPAP too!

I have been trying to find decent magnensium, Vitamin D and C without magnesium stearate at at correct doses. Very limited supplies on Amazon and most have fillers, just trying to find as natural as possible. The Vitamin D doses are also quite small.

I have never had coeliac test done so will definitely request that before starting gluten free.

You need to eat high levels of gluten before test

Blood test should be MAXIMUM of 12 hours after last dose T3 or NDT

Vitamin B complex might be better option than just Thiamine

Many thyroid patients are low on mix of B vitamins

Essential to regularly retest vitamin D, folate, ferritin and B12

Low B vitamins

supplementing a good quality daily vitamin B complex, one with folate in (not folic acid) may be beneficial.

This can help keep all B vitamins in balance and will help improve B12 levels too

B vitamins best taken after breakfast

Igennus Super B is good quality and cheap vitamin B complex. Contains folate. Full dose is two tablets per day. Many/most people may only need one tablet per day. Certainly only start on one per day (or even half tablet per day for first couple of weeks)

Or Thorne Basic B is another option that contain folate, but is large capsule

IMPORTANT...If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

When was vitamin D last tested

How much vitamin D are you currently taking

Aiming for vitamin D at least around 80nmol and around 100nmol maybe better

Better You vitamin D has no magnesium supplements in it

What are most recent results and ranges for Ferritin, Folate and B12 ?

Have you had cortisol and DHEA levels tested

Before considering getting cortisol tested....I would get all vitamins optimal and trial strictly gluten free diet

Best cortisol testing is Regenerus (or Geoneva)

I had my Vitamin D tested in Jan 2019 when it was 130, not sincec hough. My cortisol was tested last in 2018 and was 540. I am taking a B12 spray, two sprays a day. Last Ferritin test 202 (13 - 150) 2018, B12 1544 Dec 2018, folate 10.1 (sorry no ranges quoted - normal according to notes! TPO normal 2017. Dont think I was supplementing with Bs in 2018, not 100%sure though. Cortisol was 525 in 2018. None of these hqve been checked since. I was supplementing with folate a few months back but no test since. Not had DHEA tested either.

The spray from BetterYou seems quite a low dose, but I dont think Im short in it. Have been sitting in sun even this last week in garden, no suncream needed!

Icam aware of biotin re bloods and using folate and not folic acid. I guess my b12 test implied inflammation so may be should not be supplementing with Bs. I gather no point in B12 blood test if supplementing, how 9ng would I need to leave that off before testing again please? Im also aware theserum blood test is not a great indicator as it doesnt tell us what is in the cells?
