Help and guidance on blood results

So I’ve had my bloods done, I’m currently on 175mcg wockhardt Levo. I take all vitamins daily. I’m feeling very tired and struggling to wake up in the mornings.

Vit D 65.6

TSH 1.92 (0.38-5.50)

T4 11.8 (10.00-18.70)

T3 (4.6 (3.50-6.50)

Serum iron 13 (9.00-30.00)

Serum ferritin 120 (10.00-291.00)

B12 485 (211.00-911.00)

Serum folate 11.5 (5.40-20.00)

Copper 0.26 (0.20-0.60

So I’ve got liothyronine ready to take, if I need it? How much? I’m very sensitive to it but this is a new brand so hoping to feel better. But I am scared after last time.

If I did not take liothyronine and stayed on Levo only, how much should I increase? Last time I upped Levo my t4 and t3 results went lower.

Your Ft4 is still very low

Would suggest you try 25mcg increase in levothyroxine first

Any vitamin D test result?

B12 rather low

humanbean may comment on low iron levels...despite good ferritin

Vitamin D is at the top, no ranges given by gp but it’s in a similar place to last time and I supplement.

Always had problems with iron and gps always say normal even though I’m never in a good place.

So would you introduce the t3 at all and leave the Levo alone? Or do you think Levo first of all? I do want to try it again, despite the side effects my head literally felt less fog, like someone turned the light on. I’m so tired and everything that comes with feeling tired. It’s like I’m hungover or never slept. Been starting a few heart palps too and before the bloods I new things were not functioning as well.

Low copper and low iron often occur together. I'd not be surprised if you also have low zinc and other minerals. The cause is probably low stomach acid (low thyroid hormone obviously) and poor enzyme production due to genetic factors, food intolerances, SIBO or Candida, plus liver problems. Address gut issues as a priority.

Are you gluten / dairy intolerant? Dairy can present a problem in absorbing many minerals, especially iron.

You could try taking Betaine HCl prior to the largest protein containing meal of the day (minerals are usually found in meat and vegetable proteins). You may also consider taking digestive enzymes after your meals. Look at Dr Sarah Myhill's website on supporting digestion.

High Ferritin / Iron ratio suggests you've got inflammation due to chronic illness (such as arthritis). It's worth asking your GP to test for inflammatory markers. Magnesium is a good anti inflammatory, also vitamins D and C.

Hi there,

I’ve had digestive issues most of my adult life. I was classed as having ibs for years, had lots of invasive treatments. I’ve now found out I have histimine Intolerance.

I take vitamin c, d I take quercetin which has bromelian. I take magnesium, zinc and selenium at bedtime, I also take iron and good b complex.

I did take cooper for a short time but was recommended on here that most with under active thyroid don’t need copper.

My diet is gluten and dairy free and very healthy. I use lots of digestive friendly herbs, teas etc.

I believe I’ve had anti inflammatory tests before and all normal.

I am simply trying to help myself but still feel utterly rubbish. Diets for histamine intolerance often conflict with a thyroid healthy diet. :(

My daughter has the same problem with histamine and takes nettle, quercetin, ester C complex and minerals as you do. Low histamine diets are sometimes low in good fats, so make sure you're getting some vitamins E and A and Omega oils. Curries can be good (I use coconut oil in them) as turmeric is a histamine blocker - it's also an iron blocker though! Best to have an iron rich breakfast - haem iron is the easiest to absorb, but sources such as black pudding may be higher in histamine.

Obviously you're undermedicated for your thyroid, but if fatigue is your main problem it'd be worth looking into adrenal support as well, especially in the context of high histamine levels, as low cortisol allows histamine to build up.

I also use nettle and various other herbs. Turmeric made me very bloated I decided to stop taking it as it can cause gall stones in excessive consumption and I’ve had my gallbladder removed.

I’m just finishing off solgar quercetin and that has Ester c complex in it.

Which brands dose your daughter use?

The same brand- Solgar for quercetin, Ester C and nettle.

My feeling though is that given your exhaustedness that you have low cortisol which is making your histamine symptoms worse (see previous link). Ask to be referred to an Endocrinologist or get private saliva 24 hour tests for adrenals. Some forum members take hydrocortisone for low cortisol.

If you look at Dr Myhill's website she recommends CFS sufferers take adrenal support in addition to thyroid support.

I had a test already and told normal, had it twice now. I don’t have the results though for that.

Been tired most of my life, like I’m not really here. I take care of myself and know a lot but always trying to learn more.

Have you been tested for systemic Candida? Candida dieback is something I do understand to cause horrible brain fog due to acetaldehyde. The best thing for that is soluble Magnesium which neutralizes the effects of the toxins produced by Candida. Also, liver support such as NAC (helps support the liver making enzymes) can be really beneficial for brain fog symptoms that originate in the gut.

Also, you say it's been going on for a lifetime, which suggests a genetic issue such as mast cell activation disease. This is something that can be tested by your Endo.

Given your gut and absorption issues I'd be inclined to use soluble forms of Magnesium Citrate or Chloride. The most absorbable capsule form is probably Glycinate.

It's a blend of Oxide, Citrate and Glycinate, but it doesn't state the actual ratio. I wouldn't touch the Oxide form with a barge pole it's notoriously difficult to absorb. See Dr Carolyn Dean's blogs on Magnesium.

I did read the articles and a few others but the magnesium recommended has iodine in it plus on amazon it’s like £70. What supplement of magnesium would you recommend?

I really think you should ask to have a test for Mast Cell Disease. We have it in my husband's family. As well as Hashi's it occurs in families with MS.

I dare not touch black pudding and most things iron rich are high triggers. I have massive attacks. My condition is worse these last few years, my house is mouldy and that dose not help.

Any tips be nice to here from you.

Presumably you have lactose free levothyroxine?

Teva, Aristo or Glenmark

No wonder you need high dose levothyroxine

Likely to need addition of small doses of T3 alongside levothyroxine in the end...but suggest you increase levothyroxine first

Yes it is free of both but I take wockhardt as Teva made me ill and I did not seem to be as responsive on the same dose.

Aristo, no chemist I know has heard of it. I have to be carful of fillers due to histamine intolerance.

Obviously increasing my Levo means more lactose as they only come in 25mcg and I take 7 of these.

It’s Thybon Henning so lactose free. Liquid Levo made me feel ill and ranges were not good. It’s got some nasty fillers in it.

Yes wockhardt has lactose but I’ve not had a histimine response to it, but the lactose dose not help in other ways.

According to this link :

your iron needs to be between 55% - 70% of the way through the range, which would be between 20.55 and 23.7 with the range you've given.

For many people a mid-range ferritin or slightly over helps them to feel at their best, i.e. approx 150 - 180 with the range you've given.

Do you take any iron supplements, prescribed or bought? If yes, what are they, how many times a day do you take them, and what dose are they?

I currently take sytron liquid iron from the gp 2 5ml spoonfuls a day but most the time they won’t give me it due to expense. I’ve spartone but I take 2 has little or no effect, I’ve also used solgar gentle iron. Over the counter iron dose not do much to raise my iron. I’ve been on the gp stuff 4 months and gone from 9 to 13, hardly big increase. But apparently I’m perfectly normal.

Just to give you a comparison, I was very low in iron and ferritin some years ago. I was prescribed ferrous fumarate 210mg, one tablet to be taken three times per day. A single tablet of this supplement contains 69mg of iron. And I was taking three per day, spread out over the day.

I absorb iron very poorly. It took me nearly two years to raise my ferritin to mid-range. Even then my serum iron was low in range (but at least it was in range, which it wasn't at the start).

I now handle everything to do with my iron myself. I pay for iron panels and I buy my own supplements. I now take a maintenance dose. I started doing all this after that first prescription from the doctor. I knew she would think one prescription was enough, and I knew it wasn't!

I would guess that I have been low in iron or iron deficient all my life. I'm not going to let doctors leave me with low iron/ferritin ever again.

It sounds like your doctor has been much too tentative in prescribing iron supplements for you.

You'll have to decide for yourself which supplement appeals to you. The prescription-strength iron supplements aren't terribly expensive, and the ferrous fumarate 210mg comes in boxes of 84, and I would guess they work out the cheapest.
