Daughter's latest blood test results

Hello everyone,

I have my daughter's recent blood results and wanted to run them by you knowledgable people before we see her new paediatric endo next week.

She was diagnosed with M.E. 4 years ago and now on this dose of 100mcg thyroxine does seem to be a little better - she is getting restless which I'm hoping is a good sign! She however still is on her bed most of each day, and looking at her results, it seems her dose does need to be increased. She is quite tall and weighs about 60kg. She is on 210g ferrous fumarate twice daily too, and vit D, but her levels are still on the low end. She forgets everything unless I constantly remind her so cognitively she is not much better. Her appetite is poor also and she says she is just not hungry. Her hair has thinned a bit too.

Her liver function results don't look good either. Not sure what this means. She is 17yrs old. Searching on the internet it says it could be inflammation of the liver or viral hepatitis.

I'm wondering if I also now need to take her to an M.E. specialist who would prescribe anti-virals. Unless the hospital could do this next week. She did have a test for hep C but it was negative.

Please can someone explain what is going on? I am quite anxious about her, got thyroid issues myself, and on my own with all of this.

Thank you.

P.S. Couldn't send the liver results so here they are:

Bilirubin 7 (<21)

ALT 39 (10 - 35)

AST 95 (<38)

Albumin 49 (32 - 45)

Previous post shows she has autoimmune thyroid disease...Hashimoto’s

healthunlocked.com/thyroidu...

Her Ft3 is still rather low

Improving low vitamin levels may help improve conversion of Ft4 to Ft3

Exactly what vitamin supplements is she currently taking?

Folate is far too low

And ferritin too low as well

Is she on strictly gluten free diet...or tried it?

She only takes the iron and vit D atm. I will get her on my vit B complex with folate.

I doubt the endo will prescribe T3 - we live in hope- but due to her Sx he may increase her thyroxine dose - or am I being too hopeful!

She did gluten free for a while but not strictly. I will try and get her on my strict gluten free diet.

Supplementing a good quality daily vitamin B complex, one with folate in not folic acid may be beneficial.

chriskresser.com/folate-vs-...

Many Hashimoto’s patients have MTHFR gene variation and can have trouble processing folic acid.

B vitamins best taken after breakfast

Igennus Super B is good quality and cheap vitamin B complex. Contains folate. Full dose is two tablets per day. Many/most people may only need one tablet per day. Certainly only start on one per day (or even half tablet per day for first couple of weeks)

Or Thorne Basic B or jarrow B-right are other options that contain folate, but both are large capsules

If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before any blood tests, as biotin can falsely affect test results

Low ferritin

Never supplement iron without doing full iron panel test for anaemia first

Eating iron rich foods like liver or liver pate once a week plus other red meat, pumpkin seeds and dark chocolate, plus daily orange juice or other vitamin C rich drink can help improve iron absorption

This is interesting because I have noticed that many patients with Hashimoto’s disease and hypothyroidism, start to feel worse when their ferritin drops below 80 and usually there is hair loss when it drops below 50.

Thyroid disease is as much about optimising vitamins as thyroid hormones

Yes and I'm going to increase her iron tablets. Was supposed to be 3 a day anyway.

GP should retest full iron panel test if they are prescribing iron supplements

Yes there was a full iron panel on this test.

Is she supplementing Vitamin D

Vitamin D

GP will often only prescribe to bring levels to 50nmol.

Some areas will prescribe to bring levels to 75nmol

leedsformulary.nhs.uk/docs/...

GP should advise on self supplementing if over 50nmol, but under 75nmol (but they rarely do)

NHS Guidelines on dose vitamin D required

But with Hashimoto’s, improving to around 80nmol or 100nmol by self supplementing may be better

Once you Improve level, very likely you will need on going maintenance dose to keep it there.

Test twice yearly via vitamindtest.org.uk

Vitamin D mouth spray by Better You is very effective as it avoids poor gut function. There’s a version made that also contains vitamin K2 Mk7

It’s trial and error what dose we need, with hashimoto’s we frequently need higher dose than average

Government recommends everyone supplement October to April

How much vitamin D is she supplementing

How low was it before supplementing

Pinkpetite profile image
Pinkpetite in reply to SlowDragon

It was very low, I think 17 or sth originally, that was 5 years ago. They put her on 20,000 ius every 3 weeks, I had her take it every week or 2. I am now going to get her on a 2,000ius daily one.

SlowDragon profile image
SlowDragonAdministrator in reply to Pinkpetite

This one has Vitamin K2 in as well

One spray = 1000iu

Thank you for this. She had the lower dose one so I will get her back on it and buy this one when it runs out.

How long has she been on 100mcg levothyroxine?

That is the guideline dose for her weight ....but she may need slightly higher dose or addition of small dose of T3 alongside levothyroxine

But first getting vitamins optimal and trying strictly gluten free diet and/or dairy free diet

Is she always getting the same brand of levothyroxine?

Pinkpetite profile image
Pinkpetite in reply to SlowDragon

Thank you for these replies. She's been on 100mcg for about 7 weeks. It wasn't always the same brand but I'm on it now.

I will try and get her to take all the right supplements but it's not easy as she is in a rebellious phase! Same with the diet I will try.

I'm hoping that an increase in thyroxine will help. I'm 13kg less than her and I'm on 150mcg.

I have malabsorption issues so maybe she does too.

If we have no luck with this endo we will go private from Dionne's list.

Are her liver results ok?

SlowDragon profile image
SlowDragonAdministrator in reply to Pinkpetite

Don’t know anything about liver results....ask GP

7 weeks is short ...retest again in another 6-9 weeks

See if levels are improving

Always test as early as possible in morning before eating or drinking anything other than water and last dose levothyroxine 24 hours before test

Pinkpetite profile image
Pinkpetite in reply to SlowDragon

Ok will do.

Gp usually ups the dose then re-tests in 6 to 8 weeks.

This is our first appt with the endo so we will see if he is any good.
