Following on from last post healthunlocked.com/thyroidu... my latest test results in... Must have done something wrong because they didn't do all the tests I asked for (not enough blood/too many clots/damaged blood cells...?)

TSH: [not done]

T4: 103 (66-181) nmol/L

free T4: [not done]

T3: 5 (3.1-6.8) pmol/L

TPO: [not done]

TG: 51.8 (0-115) kU/L

Vit D: 55 (50-175) nmol/L

active B12: 72 (37.5-188) pmol/L

total B12: 218 (300-569) pmol/L

folate: 26.3 (8.83-60.8) nmol/L

ferritin: 51 (30-400) ug/L

transferrin saturation: 40% (20-50)

TIBC: 59 (41-77) umol/L

UIBC: 34.8 (22.3-61.7) umol/L

Serum Iron: 23.8 (5.8-34.5) umol/L

Wow that was a lot of tests! So to recap, male, 36, diagnosed hypo in Feb, slowly increased to 100micrograms Levo, been on this dose for 2-3 months. Just changed brand back to Actavis a week ago (which previously felt better than I have on Teva/Mercury) but still feeling very low energy with occasional shortness of breath/dizziness, very emotionally fragile, less mentally sharp, a dull pins and needles type feeling (especially after taking levo), some foot pain and mild joint pain.

Having got a refund for the missed tests I'm obviously going to order them again straight away, but in the meantime would anyone be able to interpret these as to some idea of what's keeping me down or what I can do about it?

Quite a few tests not done...as you say

Vitamin D is still too low

GP will often only prescribe to bring levels to 50nmol.

Some areas will prescribe to bring levels to 75nmol

GP should advise on self supplementing if over 50nmol, but under 75nmol (but they rarely do)

But improving to around 80nmol or 100nmol by self supplementing may be better

Once you Improve level, very likely you will need on going maintenance dose to keep it there.

Vitamin D mouth spray by Better You is very effective as it avoids poor gut function. There’s a version made that also contains vitamin K2 Mk7

It’s trial and error what dose we need, with hashimoto’s we frequently need higher dose than average

Never seen this Total B12 ....Should it read Serum B12?

obviously result is below range.

But Active B12 Is just over 70.

Active B12 Under 70 is considered suspect for low B12

This is interesting because I have noticed that many patients with Hashimoto’s disease and hypothyroidism, start to feel worse when their ferritin drops below 80 and usually there is hair loss when it drops below 50.

Eating iron rich foods like liver or liver pate once a week plus other red meat, pumpkin seeds and dark chocolate, plus daily orange juice or other vitamin C rich drink can help improve iron absorption

Thank you SlowDragon! I've been gradually realising the importance of vits and diet, still have a long way to go.

The 'Total B12' is how they called it (test from Thriva): "Total B12 is the standard test used to show the level of vitamin B12 in the blood stream. This test measures ALL of the blood's content of B12." I guess that's the same as the serum B12?

When you repeat test make sure to do test as early as possible in morning before eating or drinking anything other than water and last dose levothyroxine 24 hours before test
