Should I lower tirosint or T3

I just had my blood tested

Free t4 1.5. Range 0.7-1.5

Free t3 3.5 range 2.2-4.0

Total t3. .87. Range .60-1.81

Tsh. <0.10 Range 0.350-4.94

This was on 125 tirosint and 15 t3 I was doing 17.5 for a couple of weeks but I had a lot of sweating so I dropped it to 15

In January my free t4 was 1.3 range 0.7-1.5

Free t3 was 2.2. Range 2.2-4.0

Tsh 0.05. Range 0.350-4.94

Reverse T3 23. Range 9.0-27.0 which has gone up a lot From what it was I haven’t gotten it back yet this was on 150 tirosint and 10 T3

If those were my results I'd lower the Tirosint. It's usual, when taking a combinationo f Levo and T3, for TSH to be low or suppressed, for FT4 to be lowish in range and for FT3 to be in the upper part of the range, if that is where you feel well. I'm on combination and need FT4 to be a fair way through range (I'm very unwell if it's very low in range) but I need a good level of T3.

Do you think my level of T 3 is ok I do feel that I’m fatigued and somewhat over medicated I have tirosint 125 which Im on or 75 I can get a new prescription from my doctor for 100 but in the mean time should I switch to the 75 every other day I probably won’t hear from her until at least Tuesday

If those were my results I'd be happy with the FT3 result where it is. I would be unhappy with FT4 that high and I'd reduce the Levo. If you want to reduce to 100 then take 1 x 125 one day and 1 x 75mcg the next. Levo is a storage hormone and you get no immediate effect from it, so alternating doses to give an average of 100 should be fine. I would retest 6-8 weeks after the dose change.

How are your nutrient levels - Vit D, B12, Folate and Ferritin?

In January D was 53.5 ng/ml and was taking 5000-10000 because it was 29 before that ferritin last year was 73ng/ml range 8-252

B12 was 616 range 211-911 and folate 18.1 range>5.4 Thank you for all your help

The Vit D Council/Vit D Society recommend a level of 40-60ng/ml so you were nicely within that range. If you continued with the same dose then you should retest now, even if you lowered your dose it would be a good idea to check your level anyway to see if your new dose is maintaining the good level.

Have you done anything about trying to improve this level? Ferritin is recommended to be half way through range.

B12 and folate were OK although according to an extract from the book, "Could it be B12?" by Sally M. Pacholok:

"We believe that the 'normal' serum B12 threshold needs to be raised from 200 pg/ml to at least 450 pg/ml because deficiencies begin to appear in the cerebrospinal fluid below 550".

"For brain and nervous system health and prevention of disease in older adults, serum B12 levels should be maintained near or above 1000 pg/ml."

I improved my ferritin level by eating liver regularly, this should be no more than 200g per week due to it's high Vit A content, also liver pate, black pudding, and including lots of iron rich foods in your diet

Don't consider taking an iron supplement unless you do an iron panel, if you already have a decent level of serum iron and a good saturation percentage then taking iron tablets can push your iron level even higher, too much iron is as bad as too little.
