T3 & Metavive II dose advice

Hello you lovely people! I’ve just started to make the transition between 3 x grains of 30mg metavive II and t3/ thyroxine. The endo suggested I take 25mcg thyroxine and 5mcg T3 however the last time I took even 25mcg thyroxine I felt very unwell. ( I have the DIO2 issue.)

When I was on 3 grains I felt ok but under medicated, I reduced to 2 grains and 5mcg T3 just 6 days ago and don’t feel any different so I’d like to perhaps keep to the metavive/ T3 combo as I’m scared of having a setback with thyroxine.

Do any of you know roughly how much T4 one grain of metavive would have? I know they don’t list the contents but just in case... otherwise what would any of you suggest to help lift me slightly? I want to take my time on T3 but still feeling abit tired and foggy (although nowhere near as much as before.) AND... I’ve finally started to sweat for the first time in years! Not hugely but a tiny bit which I noticed when I went for a run. Small miracle! :) thanks so much for any advice as ever 🙏🏾💜

A grain is 64.8 mg. So to clarify what you are currently taking, do you mean you are taking 3 tablets of 30 mg Metavive II, ie a total of c1.5 grains; or 3 grains, which would be twice as much?

Hi- so sorry for not being clear... I mean I’m taking 3 capsules/ tablets of 30mg metavive two ( I thought they were called grains?) Thank you for being so patient with me... I’m still learning clearly! 🙏🏾😭

No problem, we all have to learn 🙂. A grain is an old unit of mass, which seems to have hung around from Apothecary times, when referring to NDTs, and relates to a specific amount/measurement. It isn't an alternative word for tablet or capsule; and even with a prescription NDT, the tablets would be measured as 1/4 grain, 1/2 grain, 1 grain and so on. It's important to be clear though, because if you are wanting advice about swapping from one form of thyroid hormone replacement, to another, there's quite a difference between 3 x 30mg of Metavive, and 3 grains of NDT. So assuming that Metavive II has 'similar', if not exactly the same active content as a prescription NDT, you're currently getting about 57 mcg T4 and 13.5 mcg T3; and it would be quite a reduction if you follow your Endo's suggestion of 25 mcg T4 and 5 mcg T3. A low start dose of T3 might be suitable for people moving from mono T4, but you've already been ingesting more than that in your Metavive, so don't need to start so low, especially as you say you felt under-medicated on 3 tablets.

Regarding testing positive for a particular SNP of your DIO2 gene; I'm not aware that an effect can be intolerance of T4; rather that the possible poor conversion to T3 and its resulting low level would be what impacts our health.

Hi @MaisieGray do you know if 3 x metavive II is about 3 grains or is it 1.5 grains?

Hi Piggyoink

If Levothyroxine doesn't agree with you I'd stick with Metavive/T3 combo or T3 only. Hope you start to feel better soon.

Hi again Elaine - just wondering how you know how much T3 and 4 is in metavive? I'm hoping to find out how much is in Metavive III and VI as I take the bovine version.
