Latest test results hypo (Medichecks) - any comments please?

Hi all

I am on 75mcg Levothyroxine and have had a new test result which I would be grateful if you would give your view on:

TSH 1.97 (0.27-4.2)

Free thyroxine: 16.8 (12-22)

Free T3: 4.23 (3.1-6.8)

Thyroglobulin a/b 51.6 (0 - 115)

Thyroid Peroxidase a/b <9 (0-34)

All vits tested well within normal as was Ferritin.

Test done as per advice, ie no Levo for 24 hours previous, and bloods collected 10am.

Looks ok to me . Thank you.

The most important thing is how do you feel? If you feel fine, then your dose is correct. If not then you are undermedicated as the aim of a treated hypo patient generally is for TSH to be 1 or below or wherever it needs to be for FT4 and FT3 to be in the upper part of their respective reference ranges when on Levo if that is where you feel well.

I feel well, SeasideSusie. I have always had an issue with sleep but it is probably not connected, and I am gaining weight but I gather there's not much you can do about that. Sleeping better this last few days - really well. Then get day after day truly rotten. Inexplicable but par for the course. So I will let the 'sleeping dog' lie I think as far as my long-suffering Gp is concerned. Thanks.

Can you add the actual vitamin results and ranges

The thyroid levels look at little under medicated.

TSH over one when on Levothyroxine can be too high

New NHS England Liothyronine guidelines November 2018 clearly state on pages 8 & 12 that TSH should be between 0.4-1.5 when treated with just Levothyroxine

A small increase in Levothyroxine might help increase low FT3. And improve CKD

If it doesn't, then look at seeing endocrinologist for prescription for small dose of T3

Active B12 130 (37.5-188); Serujm folate 12.1 (3.89-26.8); 25CH VitD 91.6 (50-200); Ferritin 106 (13-150).

They look ok

Don't let vitamin D drop under 80nmol

Folate could be higher
