Pooling issues

Hi I’ve not posted for years ....I was on 5 grains of Thyroid S but recently bloods said high T3 so I’m currently on 2 grains and prescribed 610mg iron daily as I assume I’m pooling ...should I wait for my next bloods in 4 weeks to start increasing or stick with the 2 grains till bloods?

How high was your FT3? Dropping from 5 grains to 2 grains is far too much to lower in one go.

PS—there’s not much love for the “pooling” idea here...

My Free T3 was 12.8pmol/3.0-6.0

It’s a massive drop I know and I had 2 days of extreme tiredness...but since I’ve been fine really infact I’ve noticed nothing different at all which is weird.

When my t3 was too high with symptoms of fatigue and muscle weakness I was advised to stop my NDT for a couple of days to let the excess t3 wash out then get back to slowly decreasing. I felt better within about two days. It looks as though you are converting ok. I'd be tempted to get a private ft3 test soon to see fir myself how quickly it's dropping.

Ive just read the STTM on pooling so now get the connection you are making with iron and high T3. Umm..you need more advice on this re iron issues affecting thyroid hormone.

That is a very interesting . Can I ask you how much NDT you where on when you had your symptoms with your muscle weakness and fatigue ?

I think about three grains of nature throid at the time. I did try thyroid s about three years ago but found it much weaker than NT.

Then, I stopped my NDT altogether for three days then started again on two grains. It did the trick. Back to now, not feeling brill so will take a blood test. Been stable for a while but, long story, my go persuaded me promise to try T4 this April, which I did, after several years of self dosing on ndt. (I was around two grains NDT for a while then did well on levo plus NDT ) but agreed with gp to kick off with 150 levo. Felt ok physically but brain fog came back. My TSH was under range but ft3 under mid way so back on NDT plus levo and bracing myself for a telling off. In process of trying to hit the sweet spot again. I can attest switching had no deleterious effect. In fact, the levo was a smoother ride but it did not raise my t3 enough to a level where I experience full relief.

I omitted that they were checking bloods and lowered my to 125 levo....meltdown for me.

WOW !!! What a Journey . I was dosed on high T4 after my TT . At first it was Great . But shortly there after my symptoms started palpitations , sweating , insomnia , anxiety , panic attacks etc . I am now on NDT with T4 and together that works for me . It's interesting that you seem to do well that too .

My T4 was not converting well to T3 . I was not doing well with Synthetic T3 . So I'm now on NDT for my T3 mix with T4 .

I don't think I converted well when on levo years ago either...What dose do you take of each if you don't mind me asking

I was on 1.25g NT with 75mcg T4 . Since I still have hypo symptoms I was recently raised to 88mcg T4 with the same dose NDT . I find that Nutrients help too . Vitamin "D" /K2 , B-Complex Vitamin "C" Magnesium B-12 . Iron if I test low . I recently omitted Selenium as I did not do well with it . Journaling for me my symptoms with new doses helps me tremendously .

That sounds good advice...good reminder for me to be more disciplined. I just like to dose and go. My vit D wasn't optimal last test, so back on that. Since hypot I've developed hypoglycemia and pancreatic enzymes insufficiency,plus, and suspect I now have sorgrens (spelling) attacks all the mucus membranes and joint aches and fatigue comes with flares...so it's all a bit muddy at the moment. Right....journalling and vit level test x

We're all different, but for me the T3 ratio was too high so I tried to get it to a more physiological level. I understand euthryroid level is around 10.1 and ndt is approx 4.1.

I'd like to try slow release compounded T4/T3. Off the budget though.

Yes, I've experienced doing better on combo. Pity, as the levo is free and straight forward. Lots of data and discussion on the benefits of added T3. According to one study, T4 mono-dosing did not achieve the same serum ratio T3 as normal levels.

My free T4 was 21.1pmol/11.0-26.0 with a suppressed TSH ...normal on NDT

Blood tests were introduced along with levothyroxine. Levo is T4 alone. Therefore if you take NDT which is T4, T3, T2, T1 and calcitonin the blood tests cannot correlate at all. It is how 'we feel' on doses of NDT and adjust by 1/4 tablet (either up or down - I assume if it is 1 grain) at a time till you 'hopefully' reach a sweet spot when your symptoms are resolved.

The following doctor only took a blood test for the inititial diagnosis and prescribed NDT or T3 alone. He'd never prescribe levo.

I shall give you some links and we cannot just 'guess' or way to health so it is good you are asking questions. I kow when we are desperate we want quick answers but what suits one person doesn't always suit another and sometimes changing to another NDT or adding T3 to T4 can resolve our symptoms.

What a pity that those who deal with hormones seem not to have a clue and that is Endocrinologists and doctors who prescribe. No wonder Dr Lindner cannot take on any othe patients and that's certainly not a surprise.

OK. So your FT3 was over range which suggests simple overmedication rather than pooling. But I think you might have cut your dose too far. Personally, I might have dropped a grain (no more though - and some here will say that’s too much to drop in one go) and then tested again in a few weeks. Try 4 grains and see how you go. How did you get up to 5 grains? Did you test as you went up the grains or just go by how you felt?

I think I got there just testing for any changes ...neither felt fab or rubbish to be fair I’m on 3 grains now for the last week so not sure whether to stick with that?

I’m Hashimoto yes but I’m guessing they didn’t do that this test

No point in doing it again if you've already been diagnosed. Once you have Hashi's, it's for life.

So, have you not considered that that high FT3 is due to your Hashi's, and not your dose of NDT? Do you have any other labs on 5 grains to compare it with?

Unfortunately I don't have any and I've also posted my results on a Facebook group in which I was advised to graduate down to 1 grain and add T3 while it sorts itself out when I'm optimal with iron as I was very low in iron also...

When I started out on NDT for the second time after 120/125 Levo I followed the STTM route so stopped all Levo and started on I think half grain NDT. the aim wasto go up to 2 grains in halves then drop down to quarters. I got up to 3 and felt much better than Levo but not 100%. My first test at the hospital showed I was over medicated and a was told to stop meds for ? Days and restart with 1.75. I thought that was a huge drop! I think I only had to stop for a. Our of days but can't remember now but turned out the 1.75 was right for me! I'd missed my sweet spot as I'd gone from 1.5 to 2 grains. Been pretty good since. I'd started my thyroid journey on NDT many years before until a strike of 3+ months stopped me and I went over to Levo. To be fare I was pretty good on that but as time went on not as stable.

Thanks for all the fab ideas and advice...I'm thinking of staying around 2 for as long as possible as I know see 5 grains is really quite a lot anyway...and if I feel rubbish then increase a bit see what happens untill next bloods
