Advice re latest bloods.

Please can someone interpret my latest private blood results. These were taken early morning having not taken levo for 24 hr previously. I am currently on 75mg levo and awaiting an NHS Endo referral. Gluten and dairy free. Supplementing all B vitamins, folate, zinc, magnesium, Vitamin C. Despite this i am still feeling very unwell and am about to start AIP diet to see if this helps.

TSH 3.05 range 0.27-4.20

Free T4 15.6 range 12.0 -22.0

Reverse T3 17 range 10 - 24

Reverse T3 Ratio 15.89 range 15.01-75.00

Thyroglobulin 349.000 range 0.00 - 115.00

Thyroid Peroxidase Antibodies 9.0 range 0.00 - 34.00

I have also tested positive with the DIO2 gene for one parent.

Have your bloods not been tested for FT3. - the most important value as this is the active thyroid hormone you cells/ receptors need, and would tell along with FT4 tell you if you are converting ok? You have Hashimoto's with high TgAb so you might suffer from fluctuating thyroid hormones with peaks or 'flares' , and comparative troughs. TSH still too high needs to be 1 or under, so need raise of levothyroxin of 25 mcg and then retest in 6-8 weeks etc. Reverse T3 tests are not worth having, as rather meaningless for thyroid health- high results indicates something off but could be a result of many things from for eg low calorie diet to low iron, chronic disease to acute illness, Lyme disease to certain medications, chronic inflammation or increased cortisol ...etc. You say you are taking lots of supplements but when did you last have VitD, B12, folate, ferritin tested for optimum support of thyroid health and T4 conversion?

You have missed off the FT3 result

TSH is far too high and FT4 too low. You need 25mcg dose increase in Levothyroxine.

Official NHS guidelines saying TSH should be between 0.2 and 2.0 when on Levothyroxine

(Many of us need TSH nearer 0.2 than 2.0 to feel well)

Retesting full Thyroid levels and vitamin levels 6-8 weeks after dose increase

Always take Levo on empty stomach and then nothing apart from water for at least an hour after. Many take early morning on waking, but it may be more convenient and possibly more effective taken at bedtime

Many people find Levothyroxine brands are not interchangeable. Once you find a brand that suits you, best to make sure to only get that one at each prescription. Watch out for brand change when dose is increased or at repeat prescription

I had also had my blood taken at the doctors recently with very different results as i had taken levo so i'm not sure what the doctor will say if i return with these results. either way my THS is on the rise.

So, unsurprisingly, your FT3 is also too low. Needs to be at least 5 or 5.5 for most people

Thank you. It was your response to my previous post that made me decide to get the DIO2 test and full thyroid blood. So not converting? I have had dio2 tested and have inherited the faulty gene from one parent. So my levo needs upping? And i need T3? I have started AIP diet to see if i can get my antibodies down but for a limited period only as i choose a vegan diet. I will do it for a month and then start reintroducing. I will also consider the possibility of reintroducing eggs to my diet if needed for the protein missing from nuts and legumes. All advice welcome.

You likely need high FT4 and vitamin levels must be optimal

On vegan diet it's difficult to get enough protein or B12

Presumably you have always supplemented B12 and B complex ?

Strictly gluten free diet helps many

I'm using patches to try and get my B12 optimum is currently up from 200 to around 400 . range is up to 600. So more levo? And T3 if i can get it?

B12 sublingual lozenges (eg Jarrow) or B12 mouth spray (Better You) are easily absorbed

Also a daily good quality vitamin B complex

If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 3-5 days before any blood tests, as biotin can falsely affect test results
