Some advice on results please

I just received your latest medichecks results and would like someone who’s more knowledgeable to look at them please.

TSH 0.011 (0.27-4.2)

FT4 9.93 (12-22)

Total T4 58.7 (59-154)

FT3 5.17 (3.1-6.8)

Thyroglobulin antibody 22.4 (0-115)

Thyroid peroxidase antibodies 34.7 (0-34)

Active B12 >300 (25-165)

Folate 7.39 (2.91-50)

Vit D 59.8 (50-200)

Ferritin 95.3 (13-150)

I currently take 75mcg levothyroxine and 37.5mcg liothyronine. The last dose of T4 was approx 36 hours before as I take it as night and my last dose of T3 was 9 hours before. Is the room for and increase? I’m thinking possibly T4? Although I’m much better I do still have a few hypo symptoms especially fatigue

I’m really impressed with the antibodies as when I was diagnosed in Feb 2017 they were over 1300. I’m assuming being gluten free is helping with more than the horrendous reflux I had!

I am a bit concerned that my VIT D seems to be lowish still despite taking 5000iu daily for the past 14 months, it started at 30 so not drastically bad. I also take 2x ferrous fumerate daily (stopped 7 days before) which I’m thinking of dropping to 1 a day. I have B12 injections so was expecting that to be high but is the folate ok? I do take a b complex

Any thoughts would be gratefully received x

There is room to increase both FT4 and FT3. I take a combination of Levo and T3 and my experience has shown me that I can't function with a low FT4, I need mine above half way through range. That might be the case for you, although everyone is different. It might be worth trying an increase in Levo first, that will raise your FT4 and if you do still convert a bit (I appear to) then your FT3 should rise as well. Maybe try 25mcg and see how that goes.

The PPI wont be helping anything so hopefully you take it as far away from thyroid meds as possible. Is there a chance that the stomach ulcer will eventually heal and you may be able to come off the PPIs? My cousin had duodenal ulcers caused by taking ibuprofen for pain whilst waiting for a hip replacement. The ibuprofen was stopped, she had PPIs for a few months, later tests showed the ulcers had healed so she was able to come off the PPIs.

Folate 7.39 (2.91-50)

This is too low. Folate should be at least half way through range. Does your B Complex contain 400mcg methylfolate? If not then I would change brands. I got my folate up with Thorne Basic B.

Vit D 59.8 (50-200)

This is still too low, the Vit D Council recommends between 100-150nmol/L. Are you using an oral spray? If not then I would change over to one as it has better absorption for Hashi's patients. Look at BetterYou and they do a 3000iu dose spray, I would double dose for a while so 6000iu daily for 3 months then retest. BetterYou do a combined D3/K2-MK7 spray which will be useful if you don't want to take a separate K2.

Are you taking magnesium, this helps the body use D3.

Ferritin 95.3 (13-150)

This is a good result. If you are not vegetarian, I expect eating liver regularly will help maintain this level. No more than 200g per week. That will mean you probably wont need the ferrous fumarate, which is always difficult to fit in time-wise with other supplements.

If you don't already take it then I would supplement with selenium l-selenomethionine 200mcg daily, this will help reduce antibodies and also help with conversion of T4 to T3.

I’ll start with a 25mcg increase in levo first and recheck in 8 weeks as I do still convert apparently but just not enough. I’m not sure about coming off the ppi again as I now have to take steroids due to adrenal insufficiency but I’ve got a repeat endoscopy next week to see if it’s healed. Chances are though between that and a hiatus hernia I’ll be back on them long term. I stopped for 3 months and this happened. It’s taken at least 4 hours from all my thyroid meds though.

My Vit d is doctors best 5000iu capsules so I’ve doubled it for now as I’ve just bought 360! I’ll check again in a few weeks and if it’s not changed I’ll get the spray.

I take Igennus Super B-Complex as recommended on here. I’ve just looked and it does contain 400 mcg of folate but I also have separate folic acid so I’ll add that as well.

When I was first diagnosed you recommended selenium etc and I’ve been taking it ever since. Between that and going gluten free my antibodies have come down from over 1300 to almost within range! I was shocked.
