Blood result on 100 levo and 20 t3

Hi everyone,

I have appointment with my endo Tues, got bloods back from gp yesterday.

Free t4 11 range 8 to 22

Free t3 3.70 range 2.63 - 5.69

Tsh 0.01 range 0.35- 4.94

Still tired any thoughts.

I believe you are on a T4/T3 combo treatment. The FT4 could be low due to this. I would think a raise of the T3 dose would be appropriate as the FT3 level looks low and needs to be nearer the top of the range.

How are your levels of B12 - Folate - Ferritin - VitD ? All need to be good for you to feel less tired and for the thyroid hormones to work well.

All vitamin levels good, on iron to bring ferritin up from 39

How good is good ? Is your B12 over 500 - VitD over 100 - Folate mid-range ?

VitD could do with a nudge upwards - so supplement around 3000 IU's through the winter and then re-test.

What are you taking? Are you on your Levo/T3 combination? Your free Ts are too low. FT3 is only 35% through range. T3 is the active hormone so low FT3 will cause symptoms. Personally I need both FT4 and FT3 at the upper end of range, I am on a combination.

I am still tweaking doses, I haven't been able to find the perfect doses since lowering Levo too much and having to increase back again and also having to change from Unipharma T3 (I self source and self medicate T3) to a different brand.

I take my levo in the early hours of the morning when I need a visit to the bathroom, T3 I take in one dose before I get up, sometime around 6-8am usually. I used to split my T3 but it became such a faff having to take on an empty stomach and not have a cup of coffee that I tried a single dose and haven't noticed any difference.

You could do with increasing your T3 to try and raise FT3, then see how you feel. You might be like me and need a higher FT4 as well, it's all trial and error and what suits the individual.

JohnJoe,

TSH is suppressed but FT4 and FT3 are low in range so you are not overmedicated. I would increase Thybon to 30mcg to raise FT3. Your endo may not approve as increasing dose will further suppress TSH.

Do you have Hashimoto's also called autoimmune thyroid disease diagnosed by high thyroid antibodies?

If so, especially on T3/T4 combo we very often need to be strictly gluten free as well

Can you add you actual vitamin results and ranges

Your FT4 looks too low.

Did you endo reduce Levo when started T3? If so from what to what?

Do you take your T3 in split dose?

I tested negative for thyroid antibodies , what I find interesting is before my thyroidectomy I also had a suppressed tsh with low t4?

Do you supplement any of these.

Vitamin D is often best around 100nmol

B12 - I guess you supplement along with folate

Ferritin looks ok, but eating liver once a week should help improve further

All thyroid tests should be done as early as possible in morning and fasting and don't take Levo in the 24 hours prior to test, and T3 twelve hours prior, delay and take both straight after. This gives highest TSH, lowest FT4 and most consistent results

Is this how you did the test?

Personally I found I needed FT4 and FT3 higher in range.

Yes this is how I did testing, i think my vitamins are good.

Frees are low though and tsh is suppressed even with t4 11?

I have always had a suppressed tsh even when I had a thyroid and free. T4 was low!

A lot of questions for endo!!

Perhaps they need to consider central hypothyroidism. It's not that rare, just rarely diagnosed
