You are under medicated to have TSH over range. The goal of Levothyroxine is to restore the patient to euthyroid status. For most patients that will be when TSH is 1.0 or lower with FT4 in the upper range. FT4 needs to be in the upper range in order that sufficient T3 is converted. Most NHS labs don't test FT3 unless TSH is <0.03 because they're interested in determining hyperthyroidism not low FT3 in hypothyroid patients.

Vitamin D 12.9 is severely deficient. My GP prescribed 40,000iu D3 daily x 14 days followed by 2,000iu daily to raise my vitD from <10 to 107. When you take vitD it should be taken 4 hours away from Levothyroxine and with the fattiest meal of the day as fat aids absorption.
Ferritin is within range but a little higher than optimal possibly due to inflammation from fibromyalgia.
B12 and folate are both low in their respective ranges. Check out the link below and if you think you may have B12 deficiency contact healthunlocked.com/pasoc for advice.
