138 is a VERY high TSH, suggesting you are very hypothyroid. Are you on thyroid medication?

I do implore you to edit your image to remove personal information (other than test results).

The trivially simplistic answer is: You have gone from being possibly slightly hyperthyroid or over-medicated to being very hypothyroid/under-medicated. OR the test has been interfered with by something. There are several possibilities.

What has changed, as far as you are concerned, between August and now? Medicines? Supplements? How you feel?

Will edit! I was believed to be over medicated and completely removed from meds until they got these results back yesterday they have put me back on synthroid .25mcgs to start before I was on .50

We see, again and again, dose adjustments with the subtlety of a 300 pound hungry gorilla attacking a banana. Both up (on occasion) and down.

If you were very slightly over-medicated on 50 micrograms, well adjust a bit but only a tiny bit. For example, five days on 50 micrograms and two days on 25 micrograms. Or something like that - changing number of days on each until things look better. In the end, it is always a case of whatever dosing regime is best for you.

I shall not be in the slightest surprised if you find that you need 50 micrograms or more.

Also i feel awful the worst thing is anxiety

Not surprised with that TSH. :( Hopefully you will start to feel better soon, although 25mcg is ridiculously low dose considering you were previously on 50mcg anyway. Doctors are so clueless and really don't understand thyroid disorders at all.

Anxiety is awful. Can you ask your doctor to prescribe something to help with anxiety until the thyroid levels improve?

I agree i dont think they truly understand anxiety either i could try but he will probably make me wait it out
