If you are diagnosed with hypothyroidism you will get a medical exemption certificate x
Hi there
Once your doctor has prescribed medication T4 probably then your surgery should have a form for you to complete. You fill in your details & then the doc will fill in the rest & send it off. You should get an NHS exemption card. Please be aware that you have to renew this yourself( ie ask for the form) the NHS & your GP are not obliged to remind you. (Only added this because they used to)
Good luck X
Hi Tracyxx
I would talk to your doctor as they need to sign the claim form and send it off but see here for further info: nhsbsa.nhs.uk/HealthCosts/2...
