New blood test results. Comments please.

I have just received my results. Any feedback will be appreciated. And if I should now consider the cortisol test.I have been on Levo for 21 years but still experience hypo symptoms tho Levo has helped. Trying to get my system in balance hopefully with forum guidance and can then think about NDR. Most pressing problem is fatigue and at best inconsistent energy levels.

CRP .50 (normal range less than 5.0)

Ferritin 79.7 (20-150)

TSH 0.96 (0.27-4.20)

TS total 99.4 (64.5-142)

Free T4 18.9 (12-22)

Free T3 4.07 (3.1-6.8)

Anti-Thyroidperoxidase abs 19.8 (less than 34)

*Anti Thyroglobulin Abs High 178.7 (less than 115)* Can I reduce these?

*vitamin D (25 OH) Low 49 (Deficient below 25, Insufficient 25-50)* Presume some sunshine will be enough or advise supplement?

Vit B12 273 Insufficient 140-259 I think I should increase this.

Serum Folate 15.28 (10.4-42.4) If I need to increase this what do you recommend?

Thyroglobulin antibodies are positive for autoimmune thyroid disease (Hashimoto's) which causes 90% of hypothyroidism.  100% gluten-free diet may help reduce Hashi flares, symptoms and antibodies.

As you remain symptomatic you may find a dose increase to raise FT4 higher in range, which will in turn raise FT3 helpful.  Read Treatment Options in thyroiduk.org.uk/tuk/about_... and email louise.warvill@thyroiduk.org.uk if you would like a copy of the Pulse article to show your GP.

VitD is replete 75-200 and most people are comfortable around 100.  I would supplement 2,500iu daily for 2-3 months and then hope for plenty of sunshine and resume supplementing in October. Take D3 4 hours away from Levothyroxine.

B12 is optimal around 1,000.  Supplement 5,000mcg methylcobalamin for 2-3 months and then reduce to 1,000mcg daily maintenance dose.

Folate and ferritin are good.

Thank you Clutter. Much appreciated. I will follow your advice and check out the links for more info. Would you recommend retesting in 6 months?

Lindsay1995,

Thyroid levels should be retested 6-8 weeks after a dose adjustment.  VitD can be retested in 6 monthly or annually.
