Interpretation please 👍

Hello could someone kindly translate my results they are as follows:

Serum TSH level 3.98 mu/L

(0.35-4.50)

Se thyroid peroxide me 1300u/ML

(0-60) 

Serum free T4 level 13.7 pool/L

(11-23)

It's all gobbledegook to me so would welcome an expert eye ! 😃

It looks to me as if you have Hashimotos.  The Anti-TPO is raised as is the TSH - not over range - but certainly too high for you to feel well.  The FT4 - the hormone produced by the thyroid - is low in range - so am assuming your FT3 will also be low.

You haven't mentioned your symptoms or treatment .....

Symptoms are chronic tiredness , mind fog, anxiety and now depression ( just been signed off sick from work for two weeks) also more insidious symptoms that I have assumed were just old age / menopause such as dry eyes, brittle nails, thin eyebrows , deeper hoarse voice and sundry aches and pains to name a few!

So what is your GP doing about your treatment for the thyroid ?  I would insist on testing - B12 - FERRITIN - FOLATE - IRON - VITD.  If these are low in range you will suffer the symptoms you have mentioned along.

Treatment is needed .....

Are you taking any other medications or supplements ??

Yes 50 mg of levothyroxine - been on it for two weeks and feel worse - went back to gp who wanted to put me on anti depressants but I don't want to until thyroid problem sorted

Thyroid peroxidase antibodies are positive for autoimmune thyroid disease (Hashimoto's) which causes 90% of hypothyroidism.  100% gluten-free diet may improve symptoms and reduce antibodies.

It takes 7-10 days for Levothyroxine to be absorbed  before it starts working and in the meantime your hypothyroid symptoms will have been getting worse.  You should start feeling some improvement now but it will take up to six weeks to feel the full impact of the dose.

You should have a follow up thyroid test 6-8 weeks after starting Levothyroxine.  Arrange an early morning and fasting (water only) blood draw when TSH is highest, and take Levothyroxine after your blood draw.

Most people will find symptoms resolve after their TSH drops to around 1.0 with FT4 in the upper range but symptoms can lag a couple of months behind good biochemistry.

For maximum absorption Levothyroxine should be taken with water 1 hour before or 2 hours after food and drink, 2 hours away from other medication and supplements, and 4 hours away from calcium, iron, vitamin D supplements and oestrogen.

Sometimes people feel worse AnitaApple - before they start to improve.  Ensure you are re-tested and have an increase in dose.

Did you have the other tests done I mentioned above ?  Are you taking any supplements ?
