Should I be concerned about a PSA of 0.53 after SBRT treatment for one spot.
After 18 months of undetectable PSA (after ADT & 44

treatments) , my PSA started to rise this summer and was 2.66 at it's highest. My RO send me for a PSMA- Pet scan and he found one spot on T-10. 5 treatment with SBRT in Oct - Nov and now on Dec 6th my PSA was tested to
0.53.

/ SBRT radiation to lymph nodes last week. Since then Ive begun have early AM restricted urinary flow - Very narrow stream. Rad Onc suggested tamsulosin or ibuprophen. MO, tamsulosin. Any work around for the early AM problem that I could take overnight?

RT without HT
I could possibly stretch for private treatment like HIFU, but there are such mixed reviews, that I may be better off with the
IMRT
. The 5-treatment RT protocol with no HT at the Royal Marsden in London is maybe just financially possible, but would it be any better than

and refusing HT?

Low Testosterone and value of continuing Hormone Therapy
I underwent 25

sessions in Dec 2020. Upon completion of the

my PSA and testosterone levels were acceptably low so I chose not to undergo further treatment at that time and as of Jan.2024 I am still off any treatments.

2020 and on ADT for 30 months, currently completed a 20-month ADT vacation. Current PSA is <0.01.

Adding Docetaxel to T3b?
He has completed 39
IMRT
sessions and is 1yr into a 2 yr course of Lupron+Abiraterone. No gene mutations on his Invitae genetic testing. They didn't radiate his lymph nodes during the
IMRT
because he had testicular cancer 25 yrs ago and they assume they were radiated back then.

What Treatment - Or No Treatment Advice?
Would NHS VMAT

without HT be a good option? What should I do if Gleason 3+4 or 4+3? is WW a consideration? I know this is ahead of my biopsy, but I like to have a plan A, B etc.

radiation 24 months ago Both my MO and RO are interested in whether they have " CURED" me of Oligometastatic stage IV, Gleason 9. f-18 axumin PET scan showed only spread was to 2 adjacent Lymph nodes.

SpaceOAR Gel - Extracapsular Extension
I decided to do a radiation treatments:
IMRT
and HDR Brachy. My Radiation Oncologist says that since I have
extracapsular extension
, placing spacer Gel can push cancer cells out of high dose area. He recommended instead to do bio-degradable balloon.

[/u]: PSA dropped to nadir 0.93. [u]
2023
[/u]: Age 81. PSA rose to 5.71. PSMA-PET showed small lesion remaining in prostate and new lesion in iliac node. Had 30

for iliac node and pelvis. PSA dropped to 2.95 but rose to 3.78.

I was diagnosed in 2018 psa 87 Gleason 9 3 Mets low burden invasion to Vesicles

radiation that caused calcification damage to bladder neck that has been resolved by hbot . I’m still have frequent urination at nigh due to thickening of bladder wall.

Some hopeful test news:🙏🏻
He’s been on generic Zytiga and prednisone and Lupron and finished six weeks of
IMRT
. Three months after the
IMRT
his PSA dropped to .07 and now it’s .01. We all live from test to test and I just hope and pray the good news keeps coming. He’s only 50 years old and he’ll be 51 in August.

Treatment Update IMRT with ADT 18 months later
Follow up 18 months later. I am due for another ADT injection in December. Spoke to the Urologist and he feels 18months is enough keeping with the European standard while the Oncologist would like me to stay the course and do the full 2 years. I am in favor of 18 months also, that would be nice. Still

1st post-IMRT PSA = 2.1
I completed
IMRT
on 5/22/23. My last recorded PSA from 1/23 was 4.31. My 1st PSA draw on 8/22/23 came back 2.12. I've read and have been told that PSA doesn't go down immediately with radiation, unlike surgery, and can take up to 2 years to hit its nadir.

EBRT - Is it the right thing and what to expect?
