Dexamethasone and Enzalutamide
I have read there is a moderate interaction meaning that it decreases effectiveness of the
Enzalutamide
. Does anyone know of this and should it be of concern? Thanks as always

Anything in addition to ADT and Enzalutamide
I am currently on 3-month Lupron injections and 4 daily
Enzalutamide
tablets a day. My PSA has reduced to 0.14, as of this last week's blood tests, which is good news. I'm also prescribed Apixaban for the Deep Vein Thrombosis.

Androgen Cycling Shows Promise in Castration-Resistant Prostate Cancer
The median time to PSA progression (PSA-PFS) with
enzalutamide
increased from 3.8 months after abiraterone to 10.9 months after crossover from BAT (P=0.008).

Bipolar Androgen Therapy vs Enzalutamide in Asymptomatic Men With Castration-Resistant Metastatic Prostate Cancer
https://www.practiceupdate.com/C/114237/56?elsca1=emc_enews_topic-alert BAT-enza may be an effective therapy for patients with mCRPC with progression on abiraterone. Further research is required for optimal clinical use and to identify biomarkers to predict treatment efficacy.

Enzalutamide resistance & IKKB
"Although
enzalutamide
(ENZ) has been widely used to treat de novo or castration-resistant metastatic prostate cancer, resistance develops, and disease progression is ultimately inevitable.

Continuing discussion of Adaptive Theory-- Why does early chemotherapy result in lower response rates to Lu-177 and immunotherapy??
https://pubmed.ncbi.nlm.nih.gov/32916128/ In the article below on using early Lu-177, those that had early chemotherapy had a poorer prognosis than those that used AR agents--abiraterone or
enzalutamide
--see below: https://jnm.snmjournals.org/content/59/supplement_1/529 If radiopharmaceuticals and immunotherapy

Metformin and Androgen Receptor-Axis-Targeted (ARAT) Agents Induce Two PARP-1-Dependent Cell Death Pathways in Androgen-Sensitive PCa Cells.
This is a cell study, but it provides a rationale for using Metformin with Abiraterone/
Enzalutamide
, i.e. androgen receptor-axis-rargeted (ARAT) agents.

End of BAT; a few lessons
(it works well on
enzalutamide
too but doesn’t work well on abi). Fairly safe but good medical advice advised. In particular, If there is bone pain then you may have a torrid time. The pain is likely inflammation rather than growth of the cancer per se.

. Anyone headed in this direction?

Does anyone have any thoughts on an alternative to radiation to the spine (2nd time around)?
Currently I’m on Eligard and
Enzalutamide
. The thought is that the BAT treatment may have made the PC cells sensitive to these two drugs again. I do not and never believed PC would kill me, so I'm concerned with my longterm outlook. Thank you in advance for taking the time to read. :-)
