PSA question
I started
Degarelix
on Nov 13 and Abiraterone about 3 weeks later. Would probably have started later but traveling forced a slightly different schedule than originally planned. I just got the results of a PSA test done yesterday. It is about 11 compared to the previous one before ADT of about 115.

Developments since July 2020
However they have recommended that Instead of Monthly Dose of
Degarelix
(Firmagon), I may switch to Lupron 22.5 mg , as a once in three months dose. May I seek your comments on this please. 😀

Advice and Opinions would be great.
So we have done triple hormone therapy Bicalutamide (which we stopped a few years back)
Degarelix
and Zytiga are also not suppressing a PSA rise now which is currently 61. It has been as high as 433 about 9 years ago. We got several years out of the above. He is still asymptomatic.

Update: A choice between 2 clinical trials as of yesterday afternoon. 1. AMG 509 and 2. Cocktail of 4 immunotherapy drugs.
I will continue with monthly
Degarelix
injections. I hope to hear some opinions on the antibodies and agents involved. I will share what I find out. Thanks Philly

ADT...Eligard vs. Degarelix?
She seems to be looking at Eligard or
Degarelix
. Any comments? Other preferences based on fewer side effects balanced with effectiveness?

Can high ALP indicate therapy before bone mets are confirmed?
I am on Firmagon/
degarelix
+ Bicalutamid now. Any thoughts on whether additional treatment is warranted now, to be proactive, even though the mets are not 100 % confirmed? I understand bone mets should be hit heavy and early, but is it wrong to do so before they are fully confirmed?

1st Firmagon/Degarelix Shots.
I received my first Firmagon shots (2) on Monday 10/19/2020. Three days in and the pain is still very much present from the red swollen hard pot belly that resulted. Not sure I can face this monthly for 2-3 years, even if 1 shot going forward. Is the first treatment the worst from the large dose?

2 months Firmagon...then 30 days Salvage Radiation?
At this point...the consensus (URO, RO and MO) is to do 2 months of Firmagon/
Degarelix
ADT (starts Monday)....and THEN do 30 days of SRT...and continue ADT for 2-3 years. They say that 2 months ADT is needed to "sensitize" the cancer cells.

Orchiectomy--any considerations that might be overlooked?
It has been suggested to me that I reconsider an orchiectomy (versus first-line ADT) after opting for
degarelix
, then lupron for the last few years.

Monthly co-pay costs for Firmagon/Degarelix shots on Medicare Advantage vs Regular Medicare USA?
I start monthly Firmagon injections (2-3 years estimated) next week and my Medicare advantage program just informed me that the monthly co-pay for this drug will be $429 ($5148/year). I was under the impression that drugs administered by a doctor didn't fall under drugs but treatment...and therefore

to be completely flushed out of the body after just one injection?

Chemo failure. What next?
Since diagnosis last September have monthly
Degarelix
injections. Chemo was stopped. Oncologist phoned to say he was happy with PSA and to come back in 3 months. Covid means they don't won't face to face appointments. After the chest infection, I can hardly walk.

Any news on estrogen patches or gel in comparison to lupron or degarelix regarding cardiovascular problems, blood clots?
My husband will probably begin ADT but still concerned about cardiovascular issues, specially blood clots. What about estrogen patches or gel, any news?

Decision Matrix Conundrum
Used adjuvant ADT with
degarelix
plus estradiol patches. Approximately 5-6 months until T levels recovered. Post RT my PSA fell by about half (.25 to .15 and staying there for 3 months now).

Fenbendazole probably not working for me
PSA returned, 3.65, with cessation of ADT, so recommenced
degarelix
Dec 2015 (I had waited a couple of months while expecting to start Dr. Amato’s trial of apalutamide, which never did start due to his health and eventual death.)

Completed Trial Treatment (NCT02849990) Degarelix (Firmagon) and Apalutamide (Erleada) orally for 12 months.
Hi All, I finished the trial two weeks ago. PSA is still undetectable @ 0.06. MO recommended Lupron shot every three months as standard treatment. Starting Lupron shot this Friday and wondering what l am in for? Can someone please share the effects of Lupron short or Is this the standard treatment

Is There Still a Role for Bilateral Orchidectomy in ADT for Metastatic Prostate Cancer?
Recent Egyption study [1] comparing physical castration to
Degarelix
[Firmagon]. (
Degarelix
is a GnRH [LHRH] antagonist. Traditional chemical castration drugs such as Lupron are GnRH receptor agonists.)
