Prostate Cancer (stage IV) identified in January and Small Cell Carcinoma (Neuro endocrine) in liver in August. Any suggestions?
He was put on Abiraterone and
Degarelix
. In August, PSA test showed that PSA had dropped to 2.7 and there was decrease in size and PSMA avidity of Prostate lesion. But, PSMA PET CT in August found mildly PSMA avid lesion in liver, which was not seen in the PSMA PET CT five months back.

Increased risk of dementia with ADT, especially Degarelix?
Hi everyone, my first post! A friend sent me the below earlier. Looks worrying on the face of it. I'm not great with trial data, so would appreciate the views of those more knowledgeable, especially Tall_Allen. https://www.urotoday.com/conference-highlights/asco-2023/asco-2023-prostate-cancer/144860

PSA stabilizing to 99.30 (from 3103), should we seek additional treatment or wait?
He has a triplet therapy of
Degarelix
, Zytiga and Docetaxel, but he can't stand the pain of monthly
degarelix
injections so he's gonna switch to Tritptorelin every 3 months. I read that <1 PSA is the optimal point to reach, but also I know I can't ask for the sky when the initial PSA was so high.

Fever after the monthly degarelix shot
Hi All, My father has his
degarelix
and denosumab subcutaneous injections every 4 weeks as prescribed. Of late he has started getting fevers in the night on the day of the shot. The fever usually lasts for a day and resolves on its own.

CVD & GnRH agonists/antagonists
Buserelin, Goserelin, Leuprorelin & Triptorelin are GnRH agonists, whereas
Degarelix
is a GnRH antagonist.

degarelix
end of Aug. By 25 Oct my PSA was 0.44. On 15 Dec it was 1.99 and 18 days later on 03 Jan it was 2.84. By my Oncologist’s calculation my doubling time is 8 months.

ASCO 2023 Poster: Androgen-deprivation therapy and risk of dementia in patients with prostate cancer:Clinical outcomes from real-world data.
The highest risk was witnessed with GnRH antagonists (
degarelix
), with a 92% increased risk. Interestingly, the increased risk of dementia in ADT groups was observed only among whites but not in African Americans.
