So I have been very gradually reducing the pramipexole and upping the amitryptaline

And so far so good

I have gone from max dose down to a third of that and only very few sleepless nights

Afternoon resting is still a problem but it’s working much better than I had hoped

Great to hear you're having success with reducing the pramipexole.

It can be dufficult, but it sounds.as if you're doing really well.

I am however quite surprised that you're doing so well. Amitriptyline, like most antidepressants is renowned for making RLS worse. This can take a while however.

You perhaps need to consider reducing the amitriptyline and stopping it rather than increasing it.

If I reduce then I don’t sleep So far it’s only the increase in amitryptaline that has made me able to reduce the pramipexole

It takes a 10mg increase to allow me to drop half a pill of the pramipexole

That's OK if you're happy with that. There are some things you might consider though.

Anything you take to help with withdrawal should only be temporary unless it's something that you are going to replace the pramipexole with.

You will probably need to replace the pramipexole with something or else your RLS will return to how it was before you started on pramipexole, or, as time has passed, even worse.

Amitrityline isn't a good replacement for pramipexole, it doesn't relieve RLS symptoms, it can make them worse.

you will also become dependent on it, then will have to withdraw from that. The higher the dose you take the more dependent you'll become.

The other aspect is, withdrawal effects should settle in two weeks or so. If that's the case and you're only using it to counteract the withdrawals then there's no need to increase the dose of amitriptyline.

Is it not possible that just because it hasn’t worked for some that It could be working for me Like I said so far so good

And I am keen to be rid of the pramipexole as I’ve been told that there are links to early onset dementia if used long term

Yes it is possible. I was just making you aware of the possibility.

It's not a normal alternative to pramipexole as it's not a RLS medication, but if it helps you sleep then that's OK.

I agree that it's a good idea to get off the pramipexole, not because of the risk of dementia but because of the higher risk of augmentation.

Augmentation had already kicked in with a vengeance The dementia is my main concern now

Sorry I have misundestood. If you are suffering augmentation, then that in itself is a reason for stopping pramipexole so I presumed you hadn't got it.

I think you're doing the right thing either way.

I am am on pregablin, pramipexole & codiene, but not comming of them as working at present. Sorry don't think I can answer, but any nights without RLS is a bonus, but thought amytriptalein antaganised RLS.
