Many people coming here complain of symptoms which are on the verge of rls, but simply don't sound right.
The alternative is, in many cases, peripheral neuropathy.

I can add one thing, I recently discovered some information about NBP ( in this case meaning 3-N- Butylphthalide) which is used in China for various vascular-neurological conditions and is neuroprotective. Apparently it's provided in the form of soft capsules, but I can't track down any western source of it, apart from celery oil.

you can get NAC. (n acetyl cysteine) as a supplement, it's used by prescription as an antidote to paracetamol.poisoning. and as a mucolytic.
I could add it to my list of supplements to try along with the iron, vitamin D, vitamin B12, magnesium, potassium citrate, taurine and celery seed oil.
Going to cost a fortune and might make me rattle

I tried NAC a lot, sometimes really high doses, sometimes it helps, sometimes not at all.

Thanks for this. When I was first diagnosed with RLS I complained about pain and the neurologist argued that he did not see pain as being an indicator of RLS. At the time I was adamant that RLS was causing the pain but, after many months of waiting, I had a nerve conduction test and was told I had Small Fibre Neuropathy. SFN is a subset of Peripheral Neuropathy. I have recently learned that I am Pre Diabetic and as I am keen on avoiding diabetes if I can the diet stuff is of great interest.
It's quite easy to see why people are confused as many of the websites list pins and needles and tingling as RLS. Pins and needles and tingling is also neuropathy. I can't imagine how anyone could say definitely what is what, without having a small five biopsy to rule out neuropathy.
There are two 'hacks' I would use to distinguish them.
The urgent need to move for relief.
Do Paracetamol/Ibuprofen relieve the symptoms? If they do, then unlikely to be rls.
I appreciate these are very blunt instruments, but I'd say they cover 98.35% of cases. Roughly!😅
I see that one way to tell the difference between primary RLS and Secondary RLS due to Small Fibre Neuropathy (SFN) is sensitivity to hot and cold.
Someone with the neuropathy will be less sensitive to hot and cold.

Both will have RLS symptoms however.
There are many other causes of neuropathy however, but I imagine (though have found no evidence as yet), that RLS caused by some neuropathies might not so readily respond to dopaminergic agents.
It's complicated because just because someone has secondary RLS due to a neuropathy, it doesn't mean they don't have primary RLS, (as well).
I always thought I had rls but Dr did not agree. She started me on amitriptyline 20mg night and I am fine since then.
Not RLS then, Amitriptyline makes RLS worse.

A constant pins and needles feeling in the legs that never really seemed to disappear? It makes the legs super uncomfortable
