Thank you, TA. Very informative. I am Stage IV, with spread to the regional lymph nodes discovered during surgery. I have heard conflicting comments as to whether that truly qualifies as "metastatic" or not. If not, and since it is not "local," what is the appropriate category and what NCCN guidelines apply?

It is called "regional'- stage pN1 (the "p" means it was discovered during surgery). It is metastatic, but it can often hang around the pelvic lymph nodes for quite a long time. NCCN guidelines for regional PCa apply.

I agree, especially for guidelines for Intermediate Risk with life expectancy <=10 years which differed before. Now "Observation" vs is preferred over AS or treatment.

Very helpful, but doesn't include brachytherapy in the discussion of treatment options.

They just use the term "radiation" to cover all the different kinds, but they do mention brachytherapy specifically in #32 and #37.
