Typical PSA Level Decay Post-SBRT?

Greetings Group!

I'm wondering if anyone has come across some charts or tables showing expected reduction in PSA levels following SBRT? My last PSA, pre-Tx was 8.1. At the first PSA test at 3 months post-Tx it was 3.7. I think that is a reasonable drop but not sure if it is good, bad, or indifferent. Also curious what I should expect at the 6 mo., 1 yr, etc.?

Congratulations! Down is good, but bounces are good too. You're in it for the long haul. It can be 3-4 years before it stabilizes, as it did for me.

My PSA peaked at 7.2 prior to SBRT. It has declined with each 6 month test over the past three years, reaching about .3 at last test. Not sure what NADIR will be and not a plummet, but nice steady decline that, for me, has provided some reassurance of efficacy of treatment and lifestyle changes. Hope it holds. Still nervous with each recheck. Good luck

The above graph is a composite of many patients with different initial disease status.

AFAIK, you haven't stated your initial pathology. Therefore, your 'curve' may look different from the one shown above. In my opinion what matters is that your PSA goes down and then stabilizes at some reasonably low value. As others have written, bounces are common (and nerve wrecking).

With a mean PSA of 0.01 at 42 months, I am skeptical that chart is based on men with only SBRT treatment. Perhaps ADT included?

Hi Red. Quite correct, there were 45 men in the study and of those 17 also had ADT. The original paper with the details was referenced in my first post. The study is also quite old at 2010 and based on data from a study of patients from 2006 - 2009. I referenced the study as the only example I found that had a chart of PSA decline over time following SBRT. I suspect there more modern studies but I have not come across them. Cheers!

I am 74 y.o. and I was diagnosed in 2020 G(4+3=7)Grade 3 and PSA 16.7 µg/L.I was put on Lupron 22.5 mg/12weeks X2 in 2020 June and VMAT-RT 3Gy X 20 fx in June too.

My PSA dropped to 0.18 µg/L in July 2020, then 0.03 µg/L n September 2020.

Nadir <0.01µg/L in February 2021. Last PSA 0.03µg/L in September 2021.

Thanks for the data point, Doc. That is a staggeringly dramatic drop in PSA to go from 16.7 to 0.18 in 30 days following ADT & RT! I'm no doc myself but that sounds amazing. Good for you!
