questions for the urologist when we talk about libido, sexual dysfunction and all that fun stuff

Hello again

I made an appointment to speak with the Urologist about Libido, lack of erection, orgasm etc etc but that was before I found this site. From some of what you have written or commented on I realize that there is a lot I have not tried except Viagra .

Any suggestions from any of you about what questions to bring to the appointment would be greatly appreciated

My dr had me purchase Viagra from Canada at around a couple of bucks a day to take 3 times a week. I wake up with an erection those days and it Keeps my “friend” from shriveling up and dying. I can use it for sex too (which I do occasionally) but not a ton of interest. Lol

From my experience talking to doctors I have learned that what you say can sway how they treat you. I always go to the appointments with lots of knowledge from Dr. Google. My advice is just to tell him what is bothering you and let him tell you what he thinks. Then if there is any difference with what you learned on your own bring that up in a respectful manner. This will go a long way in helping you relationship with the doctor.

I would recommend asking about bimix or trimix injections. The process of injecting yourself takes some getting use to for sure, but I have found the results to be satisfactory. Also, you might ask about continuing with a daily regiment of viagra, or the generic sildenifil, which you can but cheaply at Costco.

The standard sequence of erectile aids (from least-invasive to most-invasive):

. . . Viagra / Cialis / etc (which will only work if your erectile nerves are more-or-less OK);

. . . . . Vacuum pump (= Vacuum Erection Device);

. . . . . . . Injections (bi-mix, tri-mix) (there are several drugs, with multiple names for each);

. . . . . . . . Implant (usually not done until two years or more after surgery, when all

. . . . . . . . . else has failed).

And while figuring out which one works, you should probably be doing "penile rehab", with VED and/or pump and/or injections.

So:

. . . get a schedule for "penile rehab" activities, to keep your penis from shrinking;

. . . . . Ask for some alternative oral drugs, if you've only tried Viagra (and get precise dosing instructions);

. . . . . . Ask what VED's he recommends;

. . . . . . . Ask how long you should wait (if oral drugs and VED fails), before starting injections.

This all assumes that you've figured out that it's possible to have an orgasm without an erection. If not, discuss that, as well.

You might want to bring your wife or partner to take notes.

One more thing:

If you've had surgery or radiation, and no ADT, your libido is very likely OK -- it's "erectile function" that's a problem.

If you're on ADT, "libido" becomes an important concern.

The joke (not funny at all):

That's a whole other problem. Wassersug's ADT book has a section on it.

Ask about a rX for the 20mg sildenafil (Viagra by another name) tablets that can often be bought via GoodRX or similar apps for pennies on the dollar. I would get a bottle of 90 for under $30-40 at Safeway, for instance. My uro recommended 1 a day for improved blood flow. It did work when I wanted an erection, or at least a partial one (80-100mg), but I have given up and just masturbate soft. I still get orgasms. Anyway, ask him about that. I found libido dropped along with my erections. I just didn't care as much anymore. I still do myself every week or so and do find orgams comes quickly. The surgery really does a number on our sex life.

I have found a wide variation among urologists about how comfortable they are in taking these questions and responding to them. Some are obviously uncomfortable and want to be elsewhere. You cannot expect a helpful conversation from a physician so uncomfortable with the topic of sexual function. I know: a urologist should not have a problem taking about a urologic system function, right? Some do. If that happens to you, say something and ask if there is a colleague who might be more comfortable handling these questions. AT my last visit in the Men's Health Program it was the female urologist who was the most informed and informative. Go figure!
