This survey is completely anonymous and you will not be identifiable from your answers unless you want to be. If you are willing to take part in future consultations along similar lines you can provide your email address in the form or alternatively send a separate email to: gcasurvey2015@gmail.com

I found the survey difficult to understand, so I contacted the people who issued it and offered up this explanation of the survey. Mind I was not the only one as others have contacted me with the same problem.

They have agreed that this is exactly what they want us to think as we fill in the survey.

"Imagine you are a patient who has just been diagnosed with GCA.

Your medic now asks you a series of questions to see what is important to you in the treatment is s/he is going to give you.

So s/he asks you a series of questions and wants you to score them, depending on how you answer, will depend on the way forward.

S/he wants to know what is important to you and your answers can then enable them to choose the best was forward for you.

This is team work, medical and patient working together".

I do so hope you find the survey easier to understand, I can now go and fill it in with confidence. This survey is very important.
