Sinemet and Xadago

My mom was on sinemet (25/100) 4 times a say with azilect up till September 2020. She switched to rytary 36/145 4x a day.

By October, her blood pressure was very low so we added midrodine. We also switched from azilect to xadago but after 2 weeks she had more falls so we switched back to azilect.

November - increased anxiety.

December - She got covid and her hospital stay led to multiple infections and an additional hospital stay.

January - Dehydration plus strep throat

Just switched back to sinemet. Blood pressure great but more involuntary movements and not as smooth as when on rytary.

Meet with neurologist next week.

Anyone gone back to sinemet after trying rytary?

Anyone take sinemet with xadago?

I will add hydration is a problem. She is very bad about drinking enough liquids.

I was put on azilect first mainly to slow progression which so far I believe it has. Frankly azilect alone did little for my symptoms.

I have been taking sinemet (25/100) 4 times a day with azilect for 5 years with very positive results... I take sinemet to treat my symptoms, i.e., reduce tremors.

BUT EVERYONE IF DIFFERENT. I don't know how along ago she was diagnosed or how her PD has progressed. If it has been slow, don't stop taking the azilect. Otherwise, I would suggest you listen to your neurologist if he/she is current on PD.

I've gone back to sinemet after trying rytary because rytary messed with my sleep and also was much more unpredictable for me so it seemed like I had more OFF time (though less involuntary movements). I am sleeping MUCH better in the past 3 weeks since I've been off rytary. I also felt that anxiety was a negative side effect of rytary for me (gocovri had similar effects of insomnia & anxiety for me). I have not tried xadago yet but plan to ask my neurologist about that. And hydration is super-important so anything you can do to get her to drink more water would be good!

Thank you for sharing - how long did it take for you to start sleeping better and having less anxiety after stopping rytary? How long were you on rytary?

I was on rytary for only about 3 months & because of my sleep issues, I was still on sinemet also (rytary doses earlier in the day & sinemet later in the day). I slept better & felt less anxious within 1-2 days of stopping the rytary.

Thank you for reminding me that Rytary can cause insomnia. I’m having a terrible time sleeping, and I figured well I guess it’s just Parkinson’s, but it’s probably the Rytary. I agree it causes anxiety too. I like it but it would be nice to get more sleep.😟

I just re-read your post because I just focused on your med question earlier. Your mom has been through a lot in the past few months! And that means you have also been through a lot. She is fortunate to have you as a daughter and as an advocate! I hope next week's visit with the neurologist offers some helpful suggestions for improving your mom's day-to-day life.
