I hope that what I send you will get you right. I have difficulty getting the pull test done well which is a fundamental maneuver because the right dose of thiamine determines it with its normalization. The push must be strong. I position myself that way because if the patient falls immediately I take a step forward and support him. If the patient takes a few steps back, I have time to move too. Recently we have increased the thiamine from 2 to three grams a day because the balance control was not yet perfect. You can post videos to the health site as I have permissions
my experience: After starting in the 4g level of thiamin HCI, within a very short time and for the following fifteen week my bowel movements were regular and smooth. BIG change from the previous years. Moved to 6g a day to see if that level would affect tremor. Became constipated after two weeks. I reduced back to 4g daily, now much improved. No constipation.
Thank you for your reply. I start back on Thiamine next week had to come off it for a week and then will start at 2g instead of 4g. Is there any written documentation on Dr. C work with Thiamine, I would like to show it to my neurologist. Thanks again for your help
I have a question for doctor Antonio- pull test helps ,among other things, to determine the right dose. If the patients retreat by more than 1 step - dose to be increased
- patients standing still - correct dose but ,what if the patient is having overdose- what will be the outcome of the test - will he be standing still (also meaning right dose) or retrace (meaning deficit)

Good question. If balance isn’t an issue. Is there a better test of dose level effectiveness?
Roy - the videos and description are perplexing. Everyone will step back to regain balance to some extent- so is it only those that Do not recover quickly that are “positive” for “failing” the pull test? ( maybe needing more thiamine) . Then, just a stutter step or two of “recovery” would indicate “negative” for the pull test ( hinting that thiamine dosage might be just right, all other things considered of course).
my experience and opinion is this: the pull test you see here, is very important and shows how after a month changes improving.
It is part of the visit that the doctor does that includes
interview,
complete UPDRS test including pull test,
answer to Patient questions
look at the videos of previous visits to see the changes (usually for the better).
According to all these things, that every time the doctor does, he determines the dose of thiamine and levodopa to be prescribed, which once determined remains substantially that for a long time. So it was for me in the 5 visits made by him in three years.
Dr, C wants me to do the pull test. Do I do it at the Drs office and video it on my phone? Also, is it done medicated or not. Big difference there. Do i do the same with my phone with the before starting the thiamine with the video of speaking, walking,writting? My supps should show up on July 12th. Many thanks
